APOE、ABCB1基因多态性与阿托伐他汀调脂效果的关联性研究

【摘要】:目的探索我国冠心病患者中APOE、ABCB1基因多态性与阿托伐他汀调脂效果的关联性。方法回顾性筛选2014.12-2016.7之间应用阿托伐他汀20mg/d并进行APOE、ABCB1 G2677T/A(rs2032582)、ABCB1 C3435T(rs1045642)基因检测的冠心病患者162例,按APOE、ABCB1基因型分别分为野生基因组和突变基因组。比较各组患者基线和治疗4周后甘油三酯、总胆固醇、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇水平及变化的差异,分析APOE、ABCB1基因多态性与阿托伐他汀调脂效果的关联性。结果APOEε4基因携带组基线低密度脂蛋白胆固醇水平显著高于ε3/ε3型组与ε2基因携带组(P=0.008);ε4基因携带者应用阿托伐他汀治疗后总胆固醇与低密度脂蛋白胆固醇水平显著高于ε3/ε3型患者和ε2基因携带者(P=0.012,P=0.010)APOEε2基因携带组、ε3/ε3型组、ε4基因携带组三组间甘油三酯、总胆固醇、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇变化率无显著性差异(P0.05);ABCB1 G2677T/A、ABCB1 C3435T野生纯合组、突变杂合组、突变纯合组间基线甘油三酯、总胆固醇、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇水平,以及甘油三酯、总胆固醇、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇变化率均无显著性差异(P0.05)。结论APOEε4等位基因与低密度脂蛋白胆固醇水平升高相关。APOE基因多态性可能与阿托伐他汀治疗后总胆固醇水平有关。APOE基因多态性与应用阿托伐他汀前后甘油三酯、总胆固醇、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇变化率无关。ABCB1 G2677T/A、ABCB1 C3435T基因多态性与患者基线甘油三酯、总胆固醇、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇水平以及应用阿托伐他汀前后甘油三酯、总胆固醇、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇变化率无关。
[Abstract]:Objective to explore the association between APOEA ABCB1 gene polymorphism and the effect of Atto vastatin on lipid regulation in Chinese patients with coronary heart disease.The levels and changes of triglyceride, total cholesterol, low density lipoprotein cholesterol and high density lipoprotein cholesterol were compared between baseline and 4 weeks after treatment, and the association between APOEO ABCB1 gene polymorphism and the effect of Atto vastatin on lipid regulation was analyzed.Results the baseline low density lipoprotein cholesterol level in APOE 蔚 4 gene carrying group was significantly higher than that in 蔚 3 / 蔚 3 type group and 蔚 2 gene carrying group, and the total cholesterol and low density lipoprotein cholesterol levels in 蔚 4 gene carriers were significantly higher than those in 蔚 3 / 蔚 3 and 蔚 2 gene carriers treated with Atto vastatin.In the patients with 蔚 3 / 蔚 3 type and the carriers of 蔚 2 gene, there were three groups of triglyceride, the group of 蔚 3 / 蔚 3, the group of carrying 蔚 2 gene, the group of 蔚 3 / 蔚 3, the group of 蔚 2 gene carrying.There was no significant difference in the levels of high density lipoprotein cholesterol, triglyceride, total cholesterol, low density lipoprotein cholesterol and high density lipoprotein cholesterol.Conclusion the allele of APOE 蔚 4 is associated with the elevation of low density lipoprotein cholesterol. APOE gene polymorphism may be related to the total cholesterol level after Atto vastatin therapy and to triglyceride and total cholesterol before and after Atto vastatin therapy.Low density lipoprotein cholesterol, high density lipoprotein cholesterol change rate. ABCB1 G2677T / AhABCB1 C3435T gene polymorphism and baseline triglyceride, total cholesterol, low density lipoprotein cholesterol,High density lipoprotein cholesterol level, triglyceride, total cholesterol, low density lipoprotein cholesterol and high density lipoprotein cholesterol were not related to the changes of triglyceride, total cholesterol and high density lipoprotein cholesterol before and after the treatment of Atto vastatin.
