【摘要】:目的应用xPlane技术测量肺动脉高压(PH)患者右室纵向缩短分数(LSF),并与心脏磁共振(CMR)测量右室射血分数(CMR-RVEF)比较,探讨该参数评价PH右心功能改变的临床应用价值。方法临床确诊PH患者106例。超声心动图右心功能参数包括:右心室纵向缩短分数(LSF)、三尖瓣环收缩期位移(TAPSE)、右室面积变化分数(RVFAC)及右心做功指数(RIMP)。41例患者应用CMR测量右室射血分数(RVEF-CMR)。结果 (1)LSF与TAPSE呈正相关关系,与RIMP呈负相关关系,与RVFAC呈正相关关系(P0.001)。(2)LSF_(APm)预测RVEF-CMR45%曲线下面积为0.780;以15.5%作为界值判定灵敏度62.07%、特异度91.67%。(3)将PH患者以LSF_(APm)截断值15.5%分为2组,组Ⅰ患者LSF_(APm)≥15.5%,组Ⅱ患者LSF_(APm)15.5%。组Ⅱ患者较组Ⅰ患者TAPSE及RVFAC显著降低,RIMP明显升高(P0.001)。结论 LSF能够反映PH患者右心功能减低程度,为临床评估患者右心功能情况,制定治疗方案提供一种新的诊断指标。
