开心散治疗慢性心力衰竭合并抑郁症（心脾两虚证）的临床疗效观察

【摘要】：目的:观察应用开心散治疗慢性心力衰竭合并抑郁症(心脾两虚证)的临床疗效。方法:收集2016.03-2017.03辽宁中医药大学附属第二医院心病二科符合慢性心力衰竭合并抑郁症(心脾两虚证)纳入标准的病房及门诊患者60例,按随机数字表法随机分为试验组及对照组各30例。对照组予慢性心力衰竭西医常规治疗,试验组在对照组常规治疗基础上加用开心散中药汤剂口服。28天为1疗程。两组患者均在治疗前及治疗后测定NT-pro BNP数值,采用中医症候量化积分表、抑郁量表(SDS)、明尼苏达心衰量表进行评分,观察两种治疗方案的临床治疗效果。应用SPSS17.0统计软件对数据进行分析,计数资料采用X2检验,计量资料采用T检验,等级资料采用非参数秩和检验。结果:1.中医证候疗效方面:试验组和对照组治疗后,症状均有所改善,试验组疗效明显优于对照组(P0.05)。2.改善抑郁症状方面:两组治疗后(SDS)评分均较治疗前减低,试验组减低程度明显优于对照组,差异具有统计学意义(P0.05)。3.改善心衰症状及理化指标方面:两组治疗后NT-pro BNP数值及明尼苏达心衰生活质量调查表评分均较治疗前降低,试验组减低程度明显优于对照组,差异具有统计学意义(P0.05)。结论:1.开心散能够改善慢性心力衰竭合并抑郁症(心脾两虚证)患者的抑郁状态和心衰症状,提高患者生活质量。2.对于慢性心力衰竭合并抑郁的患者,通过对抑郁症的治疗,患者的心衰症状可以得到改善,生活质量得到明显提高。
[Abstract]:Objective: to observe the clinical effect of Xinxing Powder in treating chronic heart failure with depression (deficiency of heart and spleen). Methods: a total of 60 patients in the second Department of Heart Disease of the second affiliated Hospital of Liaoning University of traditional Chinese Medicine (Liaoning University of traditional Chinese Medicine) who met the admission criteria of chronic heart failure and depression (heart and spleen deficiency syndrome) were collected. According to the method of random digital table, 30 cases were randomly divided into experimental group and control group. The control group was treated with routine western medicine for chronic heart failure, and the experimental group was treated with the traditional Chinese medicine decoction for 28 days as a course of treatment on the basis of routine treatment in the control group. The NT-pro BNP values were measured before and after treatment in both groups. The clinical effects of the two treatments were evaluated with the quantitative symptom scale of TCM, the depression scale and the Minnesota Heart failure scale. SPSS17.0 statistical software was used to analyze the data. The counting data was tested by X2 test, the measurement data by T test, and the rank data by non-parametric rank sum test. The result is 1: 1. The curative effect of TCM syndrome: after treatment, the symptoms of the experimental group and the control group were improved, and the curative effect of the experimental group was obviously better than that of the control group (P0.05. 2). Improvement of depressive symptoms: after treatment, the scores of SDSs in both groups were lower than those before treatment, and the degree of depression in the experimental group was significantly better than that in the control group, and the difference was statistically significant (P0.05. 3). In terms of improving symptoms and physical and chemical indexes of heart failure, the NT-pro BNP value and the score of Minnesota Heart failure quality of Life questionnaire were lower in the two groups after treatment than before treatment, and the degree of reduction in the trial group was obviously better than that in the control group, and the difference was statistically significant (P 0.05). Conclusion 1. Qiangsan can improve the depression state and symptoms of heart failure in patients with chronic heart failure combined with depression (deficiency of heart and spleen), and improve the quality of life of patients. 2. For patients with chronic heart failure combined with depression, the symptoms of heart failure can be improved and the quality of life can be improved by the treatment of depression.
