幽门螺杆菌感染与冠心病相关性研究及其影响因素分析

【摘要】:目的:探讨大理地区人群中幽门螺杆菌(Helicobacter pylori, Hp)感染情况与动脉粥样硬化性心脏病(Coronary atherosclerotic heart disease, CHD)的相关性,进一步探讨CHD患者HP感染与血脂、同型半胱氨酸(Homocysteine, Hcy)、超敏 C 反应蛋白(High-sensitivity c-reactive protein,hs-CRP)、胱抑素 C (Cystatin C,CysC)的关系,及探讨不同幽门螺杆菌菌株与冠心病的相关性,为研究冠心病的诊断与治疗提供理论依据。方法:根据冠脉造影的结果,将2016年1月~2016年12月在大理大学附属医院住院治疗122例冠心病患者分为冠心病HP感染组82例和冠心病非HP感染组40例。将同期健康体检的837体检者分为健康体检HP阳性感染278例和健康体检HP阴性559例。所有病例行血脂、HP、Hcy、hs-CRP、CysC等指标的检测。应用SPSS 17.0软件进行数据处理,计量资料均以x±s表示,组间比较采用t检验,计数资料比较采用卡方检验,以P0.05表示差异有统计学意义。应用多元Logistic回归分析危险因素对冠心病的影响。结果:大理地区体检人群中幽门螺杆菌总感染为33.21%,男性感染率为35.06%,女性感染率为30.93%。122例CHD患者中HP感染阳性者82例,CHD患者HP感染率为67.21%。HP感染组血清Hcy浓度明显高于非感染组。冠心病患者中HP感染组Hcy、CysC、hs-CRP、TC、TG、LDL浓度水平明显高于HP感染阴性组患者,差异有统计学意义(P0.05),而HDL低于HP感染阴性组患者。Ⅰ型Hp菌株感染患者血浆HCY和hs-CRP水平显著高于Ⅱ型Hp菌株患者,组间差异有统计学意义(P0.05)。结论:Hp感染与冠心病患者的Hcy、CysC、hs-CRP、TC、TG、LDL和HDL间存在相关性,Hp感染可能引起血脂异常、炎性反应、Hcy升高等心血管危险因素,以促使斑块的形成和斑块的不稳定,在动脉粥样硬化发生发展过程中发挥重要作用,增加心血管疾病发生风险。
[Abstract]:Objective: to investigate the relationship between Helicobacter pylori (HP) infection and coronary atherosclerotic heart disease (CHDD) in Dali population, and to explore the relationship between HP infection and blood lipid in CHD patients. The relationship between homocysteine homocysteine, Hcyn, high sensitivity c-reactive protein hs-CRPN, cystatin Cystatin CysCand the relationship between different Helicobacter pylori strains and coronary heart disease was studied, which provided a theoretical basis for the diagnosis and treatment of coronary heart disease. Methods: according to the results of coronary angiography, 122 patients with coronary heart disease were divided into HP infection group (n = 82) and non-HP infection group (n = 40) from January 2016 to December 2016. The 837 patients were divided into two groups: 278 cases of HP positive infection and 559 cases of HP negative. All the patients were tested for the serum lipids Hcyhs-CRPU CysC and so on. The data were processed by SPSS 17.0 software. Multiple Logistic regression analysis was used to analyze the risk factors of coronary heart disease (CHD). Results: the total infection rate of Helicobacter pylori was 33.21, the infection rate of male was 35.06 and the infection rate of female was 30.93.122 cases of CHD. The serum Hcy concentration of 82 patients with HP infection was significantly higher than that of non-infection group. In patients with coronary heart disease (CHD), the level of Hcyn Cys CysCnhs-CRPnTGP LDL in HP infection group was significantly higher than that in HP infection negative group.
