H型高血压患者颈动脉内中膜厚度与血清Hcy、Cyst-c、UA的相关性研究

【摘要】:目的:探讨血清同型半胱氨酸、胱抑素-c、尿酸水平与H型高血压患者颈动脉内中膜厚度的相关性,为动脉粥样硬化疾病的早期干预及治疗提供理论依据。方法:根据入选、排除标准,选取2014年12月-2015年10月青海大学附属医院入院治疗的H型高血压3级患者101例,男53例、女48例。详细记录患者病史,测量血压,身高、体重,计算体重指数,测定血浆同型半胱氨酸、胱抑素-c、尿酸,空腹血糖、甘油三酯、总胆固醇、高密度脂蛋白、低密度脂蛋白(LDL-C)、载脂蛋白A、载脂蛋白B等指标,并行颈动脉彩超检查,根据颈动脉内中膜厚度分三组:颈动脉内中膜正常组34例、颈动脉硬化组31例、颈动脉斑块组36例,并将正常组设置为对照组。检测并比较3组的血清指标,分析其与颈动脉内中膜厚度的关系。利用SPSS20.0进行统计分析。结果:1、三组间性别、年龄、吸烟史、饮酒史、病史、FBG、TG、TC、LDL-C、APOA、APOB、BMI等指标比较差异均无统计学意义(P0.05)。2、三组间Hcy、Cyst-c、UA、HDL-C的比较分别为:Hcy水平比较:斑块组对照组,硬化组对照组(P0.05);Cyst-c水平比较均为:斑块组对照组(P0.05);UA水平比较:斑块组对照组(P0.05);HDL-C水平比较:斑块组对照组,硬化组对照组(P0.05)。3、Hcy、Cyst-C、UA与cIMT的相关性分析:Hcy、Cyst-C、UA与c IMT呈正相关(0.334、0.317、0.442,P值分别为0.001、0.001、0.000);HDL-C与cIMT呈负相关(r=-0.293,P=0.003)。结论:血清Hcy、Cyst-c、UA是AS的危险因素,HDL-C是AS的保护性因子,监测血清中各项指标水平的变化,对AS疾病的早期干预及治疗具有一定的参考价值。
[Abstract]:Objective: to investigate the correlation between serum homocysteine, cystatin-cand uric acid levels and carotid intima-media thickness in patients with H type hypertension, and to provide theoretical basis for early intervention and treatment of atherosclerosis. Methods: according to the inclusion and exclusion criteria, 101 H type hypertension grade 3 patients, 53 males and 48 females, were admitted to the affiliated Hospital of Qinghai University from December 2014 to October 2015. Record the patient's history, measure blood pressure, height, weight, calculate body mass index, measure plasma homocysteine, cystatin, uric acid, fasting blood glucose, triglyceride, total cholesterol, high density lipoprotein, measure plasma homocysteine, cystatin, uric acid, fasting blood glucose, triglyceride, total cholesterol, high density lipoprotein, Low density lipoprotein (LDL-C), apolipoprotein A (apo A) and apolipoprotein B (apo B) were divided into three groups according to carotid intima-media thickness: 34 cases in normal carotid artery group, 31 cases in carotid atherosclerosis group, 36 cases in carotid plaque group, 36 cases in carotid artery plaque group, 34 cases in normal carotid artery group, 31 cases in carotid atherosclerosis group and 36 cases in carotid plaque group. The normal group was set as the control group. The serum indexes of the three groups were measured and compared, and the relationship between them and carotid intima media thickness was analyzed. SPSS20.0 was used for statistical analysis. Results there were no significant differences in sex, age, smoking history, drinking history, history of FBGG, TGN, LDL-C and APOAA APOBN BMI between the three groups (P 0.05. 2). There was no significant difference in HDL-C between the three groups. The comparison of HDL-C between the three groups was compared with that of the control group (P < 0.05). The comparison of HDL-C between the three groups was as follows: compared with the control group, there was no significant difference in HDL-C between the three groups. The comparison of the levels of Cyst-c in the sclerosing group and the control group were as follows: the comparison of the levels of P0.05U and UA in the plaque group, the comparison of the HDL-C level in the control group of the plaque group and the control group, the comparison of the levels of HDL-C in the plaque group and the control group. Correlation between Cyst-Con UA and cIMT in the sclerosing control group the positive correlation between Cyst-Con UA and c IMT was 0.334U 0.3170.442 (P = 0.001 0. 001D 0.000) and negative correlation between HDL-C and cIMT was r-0.293P = 0. 003P = 0. 003P = 0. 001 ~ 0. 000 P = 0. 003a, P = 0. 001C = 0. 003a, P = 0. 003a, P = 0. 003a, P = 0. 003a respectively. Conclusion: the serum Hcystatin Cyst-ctrocardia is the risk factor of as. HDL-C is the protective factor of as. Monitoring the changes of serum indexes is valuable for the early intervention and treatment of as disease.
