超声检查对血友病患者膝关节状况评估的应用价值研究

【摘要】：目的探讨超声检查对血友病患者膝关节状况评估的应用价值。方法收集44例血友病患者中70个膝关节行超声检查,其中11例小于18岁患者中16个膝关节完成磁共振成像(MRI)检查,同时对膝关节按照2.1版血友病关节健康评估表(HJHS)评价其健康状况,分析超声评分与年龄及出血次数的相关性,以及与MRI评分和HJHS评分之间的关系。结果血友病关节病变严重程度与血友病的严重程度无相关性(F=0.013,P=0.987),随着年龄增加、出血次数增多,膝关节病变加重。关节病变早期的滑膜增厚及软骨改变的检出,超声及MRI检查无差异(P0.05)。超声评分、MRI评分、HJHS之间存在明显相关性(P0.05)。结论超声对于血友病性关节病的早期诊断有重要意义。
[Abstract]:Objective to evaluate the value of ultrasonic examination in evaluation of knee joint status in hemophilia patients. Methods Sixty knees of 44 hemophilia patients were examined by ultrasonography, 16 of them (11 patients younger than 18 years old) completed Mr imaging (MRI). At the same time, the health status of knee joint was evaluated according to HJHS2.1 version hemophilia joint health assessment form. The relationship between ultrasound score and age, bleeding times, MRI score and HJHS score was analyzed. Results there was no correlation between the severity of hemophilia joint lesion and the severity of hemophilia. Synovial thickening and cartilage change were detected in the early stage of articular lesions. There was no difference in ultrasonography and MRI. There was a significant correlation between MRI score and HJHS (P 0.05). Conclusion Ultrasound is important for the early diagnosis of hemophilia arthrosis.
