心脏介入术支架选型辅助系统研究

【摘要】:当前,心血管类疾病已经被认为是引起人类高死亡率的主要原因之一。由于心脏介入术具有良好的治疗效果,且恢复周期快,已成为主流的治疗心血管疾病的方法。但是,人们对于心脏介入术支架的选型方法仍主要借助于二维血管造影和个人经验上,此类选型方法难以保证选型的正确性,更不利于知识的传承与寻优。因此,基于三维的心脏介入术支架选型辅助系统的相关研究就变得尤为重要。本课题基于产生式规则理论,建立了一种用于辅助医生进行选型的系统。主要实现了支架的参数化造型,支架选型以及三维环境下血管与支架的坐标匹配。以期改善医生基于二维血管造影图选型的局限性,对临床上支架选型操作技术提供支持。选型系统包含角色与权限控制、三维参数化模型库构建、辅助支架选型与三维环境支撑。以Visual C++编程环境、SQL Server2008数据库以及Open CASCADE几何内核作为主要开发工具,将各个模块进行详细设计,完成系统的总体框架设计以及功能的实现。角色与权限控制模块可以实现系统的用户分级和支架相关数据的访问权限。三维参数化模型库构建可以辅助医生直观选取各个参数的支架模型,并为后续辅助选型提供参数和实体。辅助支架选型模块以产生式规则为理论基础,通过事实库与规则库的建立,实现支架的辅助选型。整个系统建立在三维环境支撑之上,利用坐标匹配可以使支架在选定状态下通过平移、旋转以及缩放等坐标变换操作,根据确定好的狭窄位置数据,将支架匹配到血管病变狭窄部位,提供更为直观的判断依据。三维辅助支架选型系统改善了传统二维血管造影的弊端,更加直观,也是后续三维辅助介入术研究的基础。
[Abstract]:At present, cardiovascular diseases have been considered as one of the main causes of high mortality. Cardiac interventional therapy has become the mainstream method for cardiovascular disease because of its good therapeutic effect and quick recovery cycle. However, the selection of stent for cardiac interventional surgery is still mainly based on two-dimensional angiography and personal experience. It is difficult to ensure the accuracy of the selection, and it is not conducive to the inheritance and optimization of knowledge. Therefore, the research of three-dimensional stent selection assistant system for cardiac interventional surgery becomes more and more important. Based on the theory of production rules, a system is established to assist doctors to choose their type. The parameterized modeling of the stent, the selection of the stent and the coordinate matching between the blood vessel and the stent in three-dimensional environment are realized. The aim of this paper is to improve the limitation of two-dimensional angiogram selection and to provide support for clinical stent selection. The selection system includes role and authority control, 3D parameterized model base construction, auxiliary support and three-dimensional environment support. Taking the Visual C programming environment and the Open CASCADE geometry kernel as the main development tools, each module is designed in detail, and the overall frame design and the function realization of the system are completed. The role and privilege control module can realize the user grading and the access rights of the data related to the support. The 3D parameterized model library can be used to construct the scaffold model which can help doctors to select each parameter directly, and provide parameters and entities for subsequent auxiliary selection. The auxiliary support selection module is based on the production rule and realizes the auxiliary support selection through the establishment of the fact base and rule base. The whole system is based on 3D environment support, and coordinate matching can make the support operate by translation, rotation and zoom coordinate transformation in the selected state, according to the determined narrow position data. The stents were matched to the stenosed sites of vascular lesions to provide a more intuitive basis for judgment. Three-dimensional assisted stent selection system improves the disadvantages of traditional two-dimensional angiography and is more intuitive, and is also the basis of follow-up three-dimensional assisted interventional surgery.
