周围动脉疾病腔内治疗的研究进展

【摘要】:背景:周围动脉疾病(PAD)是一种严重危害身体健康,高致残率、高死亡率的疾病,随着人口老龄化和饮食结构的变化,PAD已成为全球性的问题。除了改变生活方式、严格戒烟及应用药物等保守治疗外,手术治疗是重要的治疗手段。相关技术的不断进步,腔内治疗PAD的应用和适应症越来越广泛,并且与传统外科治疗方式相比,腔内治疗有着低创伤、较高的可重复性等优势。然而支架内再狭窄(in-stent restenosis,ISR)等问题也日益突出,越来越引起人们的重视。DCB、DES等新兴技术的出现旨在以腔内治疗方式解决ISR,减少再次手术的创伤。目的:通过综述目前腔内治疗PAD及ISR等方面的研究,从而为临床上更好的治疗PAD提供一些参考。方法:以“周围动脉疾病、下肢动脉疾病、股乆动脉疾病、腔内治疗、药物洗脱支架、药物涂层球囊、支架内再狭窄”等关键词进行检索,找出腔内治疗PAD及ISR的相关研究。结果与结论:目前有大量有关PAD腔内治疗的文献,综合这些研究结果表明腔内治疗是目前治疗PAD可以首先被考虑的方式;传统腔内治疗有着较高的ISR;DCB和DES的研究旨在以腔内治疗的方式解决ISR,其治疗效果较传统腔内治疗明显提升,但仍有许多问题亟待解决。
[Abstract]:Background: peripheral artery disease (PAD) is a serious health hazard, high disability rate, high mortality disease. With the aging of population and changes in dietary structure, pad has become a global problem. In addition to lifestyle changes, strict smoking cessation and conservative treatment such as drug use, surgical treatment is an important treatment. With the development of related technology, endovascular treatment of PAD is more and more widely used and indications. Compared with the traditional surgical treatment, endovascular treatment has the advantages of low trauma and high reproducibility. However, the problems of restenosissis-in-stent (ISR) are becoming more and more prominent, and the emergence of new technologies, such as .DCBPDES, is aimed at solving ISRs by endovascular treatment and reducing the trauma caused by reoperation. Objective: to provide some references for better clinical treatment of PAD by reviewing the current studies on endovascular treatment of PAD and ISR. Methods: "peripheral artery disease, lower extremity arterial disease, femoral artery disease, intravascular therapy, drug-eluting stents, drug-coated balloon, restenosis in stents" were used to find out the related studies of intracavitary treatment of PAD and ISR. Results & conclusion: there are a lot of literatures about intracavitary treatment of PAD. These results indicate that endovascular therapy is the first method to be considered in the treatment of PAD. The traditional intracavitary therapy has higher ISR DCB and DES. The purpose of this study is to solve the problem by endovascular therapy. The therapeutic effect of traditional endovascular therapy is much better than that of traditional intracavitary therapy, but there are still many problems to be solved.
