老年男性心力衰竭患者血浆睾酮水平与其心功能及心血管事件的相关性

【摘要】：目的探讨老年男性心力衰竭患者血浆睾酮水平与其心功能及心血管事件的相关性。方法选择老年男性心力衰竭患者80例作为心力衰竭组,年龄相匹配的有基础心脏病史但未发展为心力衰竭的男性非心力衰竭患者80例作为对照组。心力衰竭组根据住院期间是否发生心血管事件分为心血管事件组22例和无心血管事件组58例。测定两组血浆睾酮水平和左室舒张末径(LVEDd)、左心室质量指数(LVMI)。结果心力衰竭组患者血浆睾酮水平明显低于对照组(P0.05),心力衰竭组LVEDd和LVMI明显高于对照组(P0.05)。心力衰竭患者血浆睾酮水平与心室重构指标LVEDd和LVMI均呈负相关(P0.05)。心力衰竭患者不同心功能分级之间血浆睾酮水平比较差异显著(P0.05),其中NYHA-Ⅲ级和Ⅳ级患者血浆睾酮水平明显低于Ⅱ级患者(P0.05),Ⅳ级患者血浆睾酮水平明显低于Ⅲ级患者(P0.05)。心血管事件组心力衰竭患者血浆睾酮水平低于无心血管事件组(P0.05)。结论老年男性心力衰竭患者血浆睾酮水平下降,可加重心室重构,血浆睾酮下降程度和心功能严重程度及心血管事件的发生有关。
[Abstract]:Objective to investigate the relationship between plasma testosterone level and cardiac function and cardiovascular events in elderly male patients with heart failure. Methods Eighty aged male patients with heart failure were selected as heart failure group and 80 age-matched non-heart failure patients with basic history of heart disease but not developing heart failure as control group. Heart failure group was divided into cardiovascular event group (22 cases) and no cardiovascular event group (58 cases) according to whether cardiovascular events occurred during hospitalization. Plasma testosterone levels, left ventricular end-diastolic diameter (LVED), left ventricular mass index (LVMI) and left ventricular mass index (LVMI) were measured in both groups. Results the levels of plasma testosterone in patients with heart failure were significantly lower than those in control group (P 0.05). LVEDd and LVMI in heart failure group were significantly higher than those in control group (P 0.05). Plasma testosterone level was negatively correlated with LVEDd and LVMI in patients with heart failure (P 0.05).
