冠心病患者宗气指数与心肺功能指标的相关性研究

【摘要】:课题来源:中国中医科学院中医基础理论研究所第十批研究生课题(课题号:YZ-1666)。目的:本课题通过探讨冠心病患者宗气指数(ZQI)与心肺运动试验产生的代谢当量(MET)、摄氧量(V02)、氧脉搏(02pulse)间的相关性,进一步验证宗气指数在评估心肺功能的意义,以期能为评价宗气理论指导冠心病诊疗的效果研究提供实证依据。方法:根据纳入标准选取北京市第一中西医结合医院的59名冠心病患者,使用头戴式宗气指数测量仪测量受试者ZQI,同时使用心肺功能仪测得MET、V02、02pulse。根据心肺运动试验程序,由"热身"至"运动"的变化节点取值MET-1、V02-1、02pulse-1,及同步获得的ZQI-1;由"运动"至"恢复"的变化节点取值MET-2、V02-2、02pulse-2,及同步获得的ZQI-2;及2个时点的数值差获得MET差值、V02差值、O2pulse差值。运用epidata数据库对数据录入与整理。运用多元线性回归方法探讨ZQI(ZQI-1、ZQI-2、ZQI差值)与同组 MET(MET-1、MET-2、MET 差值)、V02(V02-1、V02-2、V02 差值)、02pulse(02pulse-1、02pulse-2、02pulse差值)的相关性。并根据心肺功能的影响因素,探讨影响因素与模型方程中各个变量之间的相关性,对所拟合的模型进行验证。结果:一般情况:共纳入59例病例。男性46例(78.0%),女性13例(22.0%),男女比例3.54:1。年龄为29-84岁,平均年龄(60±11.58)岁。受试者的BMI介于(18.6-30.8)Kg/m2,平均值为(24.7±2.79)Kg/m2。冠心病保守治疗19例(32.2%)、PCI术后组33例(55.9%)、CABG术后组6例(10.2%),另有1例受试者曾先接受过CABG术,术后6个月接受了PCI术。受试者合并的疾病主要包括心脑血管系统疾病、呼吸系统疾病、代谢类疾病、运动相关疾病等。受试者中,近1个月来无用药记录的共3例,其余56例受试者1个月内服用1至9种不等的药物治疗,有服用β受体阻断药的共41例(69.5%),未使用β受体阻断药的18例(30.5%)。ZQI与MET、V02、02pulse的相关性分析:ZQI与V02和02pulse具有中度相关性,可拟合三组模型分别为①(ZQI-1)=85.826-0.127×(VO2-1)+9.172×(O2pulse-1)。且该模型中,V02-1 与 02pulse-1 对 ZQI-1 的作用相当。②(ZQI-2)=26.987-0.097×(VO2-2)+12.095×(O2pulse-2)。且该模型中,V02-2 对 ZQI-2 的作用较大。③(ZQI 差值)=-34.431-0.071 ×(V02差值)+8.809×(O2pulse差值)。且该模型中,V02差值对ZQI差值的作用较大。经验证,BMI与ZQI-1成负相关,与V02-1成正相关;年龄与ZQI-2成正相关,与 V02-2、O2pulse-2 成负相关。结论:①ZQI与V02、02pulse存在相关性,与MET相关性较小。ZQI可用于评估心肺功能。②本研究中,BMI和年龄为ZQI与V02的共同影响因素。
[Abstract]:Source: the tenth graduate program of Institute of basic Theory of TCM, Chinese Academy of traditional Chinese Medicine (Project No.: YZ-1666). Objective: to investigate the correlation between ZQI and metabolic equivalent (met), oxygen uptake (V02) and oxygen pulse (02pulse) in patients with coronary heart disease (CHD), and to further verify the significance of ZQI in evaluating cardiopulmonary function. In order to evaluate the effect of Zong Qi theory guiding coronary heart disease diagnosis and treatment to provide an empirical basis. Methods: according to the inclusion criteria, 59 patients with coronary heart disease were selected from Beijing first Integrated traditional Chinese and Western Medicine Hospital. ZQI was measured by using the headset Zong gas index meter, and METV02C02pulsewas measured by cardiopulmonary function instrument. According to the cardiopulmonary exercise test procedure, The change nodes from "warm up" to "motion" are valued MET-1 / V02-1 / 02pulse-1, and ZQI-1obtained simultaneously; MET-2V02-2O2pulse-2 from "motion" to "recovery", and ZQI-2obtained simultaneously; and the value difference of MET-1V02 is obtained by O2pulse difference. Use epidata database to input and organize data. The correlation between ZQI (ZQI-1ZQI-2ZQI difference) and MET-2 (MET-2MET-2MET difference) V02 (V02-1 / V02-2V02) was studied by using multivariate linear regression method, and the correlation between ZQI (ZQI) and MET (02pulse-102pulse-2pulse-2pulse) was investigated. According to the influencing factors of cardiopulmonary function, the correlation between the influencing factors and the variables in the model equation was discussed, and the fitted model was verified. Results: general situation: a total of 59 cases were included. There were 46 males (78.0%) and 13 females (22.0%). The average age was (60 卤11.58) years. The BMI of the subjects ranged from (18.6-30.8) kg / m ~ (2), with an average of (24.7 卤2.79) kg / m ~ (2). There were 19 cases (32.2%) of coronary heart disease treated conservatively, 33 cases (55.9%) in PCI group, 6 cases (10.2%) in CABG group, and 1 case (10.2%) in CABG group. One patient received CABG first and PCI was performed 6 months after PCI. The main diseases included cardiovascular and cerebrovascular diseases, respiratory diseases, metabolic diseases, exercise-related diseases and so on. Of the subjects, 3 had no record of drug use in the past month, while the remaining 56 patients took one to nine different drug treatments within one month. There were 41 cases (69.5%) who took 尾 receptor blockers, 18 cases (30.5%) without 尾 receptor blockers. The correlation between ZQI and METV0202pulse was moderate. The three groups of models were 1 (ZQI-1) 85.826-0.127 脳 (VO2-1) 9.172 脳 (O2pulse-1), the results showed that: 1 (ZQI-1) was 85.826-0.127 脳 (VO2-1) 9.172 脳 (O2pulse-1). The effect of V02-1 on ZQI-1 is equal to that of 02pulse-1 on ZQI-1. The effect of V02-1 on ZQI-1 is 26.987-0.097 脳 (VO2-2) 12.095 脳 (O2pulse-2). In this model, the effect of V02-2 on ZQI-2 is greater. 3 (ZQI difference) -34.431-0.071 脳 (V02 difference) 8.809 脳 (O2pulse difference). In this model, the effect of V02 difference on ZQI difference is significant. BMI was negatively correlated with ZQI-1 and positively correlated with V02-1, age was positively correlated with ZQI-2 and negatively correlated with V02-2O2pulse-2. Conclusion there is a correlation between V02ZQI and V02O2pulse, and a small correlation with met. ZQI can be used to evaluate the influence factors of BMI and age of ZQI and V02 in the study of cardiopulmonary function .2 in this study.
