慢性心力衰竭患者血清铁调素水平变化及临床意义

【摘要】:目的探讨铁调素在慢性心力衰竭(简称心衰)发生、发展中的作用。方法选择慢性心衰患者113例作为心衰组,同期查体健康者100例作为对照组,检测两组血清铁调素水平,分析心衰组伴贫血与不伴贫血者心功能分级、血清铁调素水平;随访1年,比较心衰组伴与不伴贫血者心源性死亡例数,存活者与死亡者入院时血清铁调素水平。结果心衰组与对照组血清铁调素水平分别为(95.10±43.37)、(27.45±5.03)ng/m L,两组比较P0.01;心衰组轻度心衰者(64例)血清铁调素水平高于重度心衰者(49例),二者比较P0.01;伴贫血者(21例)低于不伴贫血者(92例),二者比较P0.05。心衰组血清铁调素水平与心功能分级呈负相关(r=-0.258,P0.01)。随访1年,心衰组发生心源性死亡10例,其中伴贫血者5例(23.81%)、不伴贫血者5例(5.43%),二者比较P0.05。心衰组存活者(103例)与死亡者(10例)入院时血清铁调素水平分别为(106.94±20.3)、(105.10±35.80)ng/m L,二者比较P0.05。结论慢性心衰患者血清铁调素水平升高,其水平变化可能诱发心衰合并贫血,导致患者心功能下降、预后差。
[Abstract]:Objective to investigate the role of iron modulin in the pathogenesis and development of chronic heart failure (CHF). Methods 113 patients with chronic heart failure were selected as heart failure group and 100 healthy persons as control group. The number of cardiogenic deaths in patients with and without anemia and the levels of serum iron modulin in patients with and without anemia were compared after 1 year follow-up. Results the serum levels of iron modulin in heart failure group and control group were (95.10 卤43.37), (27.45 卤5.03) ng/m / L, respectively (P 0.01), while in mild heart failure group (64 cases) it was higher than that in severe heart failure group (49 cases) (P 0.01), and that in anemia group (21 cases) was lower than that in without heart failure group (21 cases). Patients with anemia (92 cases) were compared with each other (P 0.05). There was a negative correlation between serum iron levels and cardiac function grade in heart failure group (r = -0.258, P 0.01). After 1 year follow-up, 10 cases of cardiac death occurred in heart failure group, 5 cases (23.81%) with anemia and 5 cases (5.43%) without anemia. The serum levels of iron modulin were (106.94 卤20.3), (卤35.80) ng/m / L in patients with heart failure (n = 10) and (106.94 卤20.3), () / (105.10 卤35.80) ng/m / L in patients with heart failure (n = 10), respectively (P < 0.05). Conclusion the level of serum iron modulin in patients with chronic heart failure is increased, which may induce heart failure complicated with anemia, leading to the decrease of heart function and poor prognosis.
