PAPP-A、hs-CRP、sCD40L、IL-6及BNP在急性冠脉综合症中的诊断价值

【摘要】:目的:急性冠脉综合征(acute coronary syndrome,ACS)是一种严重危害人类健康的疾病,近年来研究发现炎症反应在动脉粥样硬化斑块的发生发展中起重要作用。而其中,妊娠相关血浆蛋白A(pregnancy associated plasma protein A,PAPP-A)是目前的研究热点之一。本研究通过对ACS患者PAPP-A以及ACS相关的生物标志物白细胞介素-6(interleukin-6,IL-6)、血浆高敏C反应蛋白(high sensitivity C reactive protein,hs-CRP)、可溶性白细胞分化抗原40配体(soluble CD40 ligand,sCD40L)和B型钠尿肽(brain natriuretic peptide,BNP)的检测,探讨PAPP-A在急性冠脉综合症中的临床价值及与hs-CRP、sCD40L、BNP及IL-6相关性。方法:1.本研究选取2015年12月到2016年6月蚌埠医学院第一附属医院心内科诊断为ACS的患者169名,其中男性患者116名,女性患者53名,并将其分为心肌梗死(acute myocardial infarction,AMI)组和不稳定型心绞痛(unstable angina,UA)组。另外从体检人群中选取60名健康人员作为对照组。2.所有患者均在入院次日凌晨,体检者在体检当日早晨空腹采集静脉血,经抗凝、离心分离血浆,-70℃保存待测。采用电化学发光法检测PAPP-A和IL-6;免疫比浊法检测hs-CRP;ELISA法检测sCD40L;化学发光法检测BNP。3.本次研究应用SPSS19.0进行数据储存,并分析不同组间PAPP-A及IL-6、BNP、hs-CRP、sCD40L水平的差异,当P0.05时认为差异具有统计学意义。结果:各组患者在年龄、性别、高血压病、血糖、血脂、吸烟史等因素间比较均无统计学意义(P0.05);UA组和AMI组患者的PAPP-A、IL-6、hs-CRP、sCD40L和BNP的浓度明显高于对照组(P0.05);UA组患者的hs-CRP和BNP浓度明显低于AMI组,UA组与AMI组患者的PAPP-A、IL-6和sCD40L浓度没有差异(P0.05)。ACS患者的PAPP-A水平与hs-CRP以及BNP呈正相关(P0.05),但是与IL-6和sCD40L无相关性(P0.05)。结论:1.在急性冠脉综合征患者血液中PAPP-A、IL-6、hs-CRP、sCD40L和BNP水平显著升高;2.不稳定性心绞痛患者血液中hs-CRP和BNP水平显著低于急性心肌梗死患者,不稳定性心绞痛患者和急性心肌梗死患者血液中PAPP-A、IL-6和sCD40L水平没有明显差异;3.急性冠脉综合征患者的PAPP-A浓度与hs-CRP和BNP呈正相关,但是与IL-6和sCD40L无相关性。

Among them, the pregnancy associated plasma protein A (pregnancy associated plasma protein Agna PAPP A is one of the hot research topics at present. From December 2015 to June 2016, 169 patients were diagnosed as ACS in Department of Cardiology, first affiliated Hospital of Bengbu Medical College, including 116 male patients and 53 female patients. They were divided into (acute myocardial infarction group and unstable angina pectoris group. In addition, 60 healthy people were selected as the control group from the physical examination population. 2. All the patients were in the early hours of the next day after admission. The patients collected venous blood on an empty stomach in the morning of the physical examination. The plasma was separated by anticoagulation and centrifuged to be stored at -70 鈩,
