射血分数保留与射血分数降低心衰的生物学标记物的对比研究

【摘要】:目的:观察可溶性ST2(sST2)、胱抑素C(Cys C)、C反应蛋白(CRP)、心脏肌钙蛋白T、I(cTnT、I)及B型脑钠肽(BNP)在射血分数保留与射血分数降低心衰患者中的表达水平,探讨其与左室射血分数的相关性。方法:选取在2015年12月至2017年2月在新疆自治区人民医院医院心内科以心衰为主要原因住院的且大于18岁的患者86例。心衰诊断标准参照2014中国心力衰竭诊断和治疗指南,由2名心内科医师确诊,依据LVEF分为LVEF降低的心衰和LVEF保留的心衰,同期住院心功能正常者40例作为正常对照组。测定实验所需心衰标志物指标,并收集一般临床特征资料,对所得结果进行分析。结果:三组人群一般临床特征资料比较:年龄、性别、BMI、DBP在三组中差异无统计学意义;HF-PEF组和HF-REF组比较,HF-REF组的sST2、cTnT、cTnI、Cys C及BNP的表达水平较高,差异有统计学意义,hs-CRP在HF组之间差异无统计学意义;比较心衰标记物与LVEF的相关性,BNP、sST2、cTnT、cTnI、Hs-CRP、CysC与LVEF均有相关性,相关性具有统计学意义。结论:1.sST2、cTnT、cTnI、Cys C及BNP等可作为区分HF-PEF和HF-REF的标志物,有重要参考价值。不同心衰标记物表达水平的差异提示HF-PEF和HF-REF在病理生理机制中可能存在不同。2.BNP、sST2、cTnT与LVEF表达水平与左室射血分数相关关系较密切。
[Abstract]:Aim: to investigate the expression levels of soluble ST2 (sST2), cystatin C (Cys C) C-reactive protein (CRP), (CRP),) and type B brain natriuretic peptide (BNP) (BNP) in patients with heart failure (HF), and to explore the relationship between them and left ventricular ejection fraction (LVEF). Methods: from December 2015 to February 2017, 86 patients over 18 years old who were hospitalized in the Department of Cardiology of Xinjiang Autonomous region people's Hospital for the main cause of heart failure were selected. According to 2014 guidelines for the diagnosis and treatment of heart failure in China, the diagnostic criteria for heart failure were confirmed by two cardiologists. According to LVEF, the patients were divided into two groups: LVEF decreased heart failure and LVEF reserved heart failure. 40 patients with normal cardiac function in the same period were taken as normal control group. The necessary markers of heart failure were measured and the general clinical data were collected to analyze the results. Results: there was no significant difference in age, sex and BMI-DBP between the three groups. The expression of sST2cTnTnTnTnTnTnTnCys C and BNP in HF-REF group was higher than that in HF-REF group. There was no significant difference in hs-CRP between HF group and the correlation between BNPs ST2TnTnTnTnTnTnTnTnHs-CRPnCysC and LVEF. ConclusionsST2, cTnTnTnTnTnTnCys C and BNP can be used as markers to distinguish HF-PEF from HF-REF, which has important reference value. The differences of expression levels of markers in different heart failure suggest that HF-PEF and HF-REF may have different pathophysiological mechanisms. 2. The expression of BNPs ST2cTnT and LVEF is closely related to left ventricular ejection fraction (LVEF).
