社区高血压患者血压控制效果的社会决定因素

【摘要】：目的:探讨社区高血压患者血压控制效果的社会决定因素。方法:选取上海市社区卫生服务中心2014年1月-2015年5月纳入管理的791名高血压患者进行面对面问卷调查。结果采用卡方检验和多因素Logistic回归分析。结果:有效问卷772份,有效应答率97.6%,血压控制良好率为52.0%。多因素分析显示,高血压患者血压控制效果受性别(OR=1.394,95%CI:1.018~1.908)、有无伴侣(OR=1.714,95%CI:1.011~2.903)等因素的影响。结论:性别、有无伴侣是社区高血压患者血压控制效果的社会决定因素,社区应高度关注男性及无伴侣高血压患者的健康教育及生活方式干预,并通过开展同伴教育、发挥伴侣的积极影响、设置咨询点(站)等不断改善高血压患者的生命质量。
[Abstract]:Objective: to explore the social determinants of blood pressure control effect in community hypertensive patients. Methods: 791 hypertension patients in Shanghai Community Health Service Center from January 2014 to May 2015 were investigated with face to face questionnaire. Results chi-square test and multivariate Logistic regression analysis were used. Results: there were 772 valid questionnaires, the effective response rate was 97.6, and the blood pressure control rate was 52.0. Multivariate analysis showed that the blood pressure control effect in hypertensive patients was influenced by sex (OR1. 39495 CI: 1.018 / 1.908) and with or without a partner (OR1. 714 / 95 CI: 1.011 / 2.903). Conclusion: sex and partner are the social determinants of blood pressure control effect in community hypertension patients. The community should pay close attention to health education and lifestyle intervention of male and non-partner hypertensive patients, and carry out peer education. Give full play to the positive impact of partners, set up counseling points (stations) and continue to improve the quality of life of patients with hypertension.
