踝—臂脉搏波传导速度与冠心病的相关性研究

【摘要】:目的探讨踝-臂脉搏波传导速度(ba PWV)与冠心病的相关性方法选取2014年01月至2014年12月期间,在福建医科大学附属二院心内科行冠状动脉造影以及ba PWV检查患者119例。根据冠状动脉造影结果分为CHD组(56例)和对照组(63例),同时将CHD组进一步分为单支病变组(21例),双支病变(17例),严重病变组(18例),比较冠心病各组与对照组的ba PWV水平,并对冠脉狭窄程度的评分(Gensini评分)与ba PWV水平作直线相关分析。以冠状动脉造影为金标准,绘制ROC曲线评价ba PWV值对CHD的筛选价值。结果CHD组ba PWV显著高于对照组(P0.05),进一步把冠心病组分成单支病变、双支病变、严重病变组,所对应的ba PWV值分别为:1558.43±97.26cm/s、1698.00±100.49cm/s、1892.56±209.30cm/s,两两比较均有统计学意义(P0.05)。Ba PWV水平与Gensini积分作直线相关分析结果显示呈正相关(r=0.886,P0.01)。由ROC曲线特征发现,ba PWV的最佳截点为1525cm/s,此时Yuedden指数为0.728,敏感性83.9%,特异性88.9%。以ba PWV1525cm/s为ba PWV异常增高为因变量,做二项分类Logistic回归分析,结果提示年龄(OR值1.112)、高血压(OR值3.183)、糖尿病(OR值2.953)、HCY(OR值3.091)是ba PWV异常增高独立危险因素。以冠心病有无为因变量,做二项分类Logistic回归分析,结果显示高血压、糖尿病、ba PWV水平、吸烟、HCY是CHD独立影响因素,其OR值分别为6.813、5.375、78.247、5.932、4.765。结论CHD与ba PWV等危险因素呈正相关关系。ba PWV对筛选CHD有一定的价值,并能够预测冠脉病变的严重程度。
[Abstract]:Objective to investigate the correlation between ankle brachial pulse wave velocity (ba PWV) and coronary heart disease (CHD) methods 119 patients with coronary artery disease underwent coronary angiography and ba PWV examination from January 2014 to December 2014 in Department of Cardiology affiliated to Fujian Medical University. According to the results of coronary angiography, the patients were divided into CHD group (56 cases) and control group (63 cases). The CHD group was further divided into single vessel lesion group (21 cases), double vessel lesion group (17 cases) and severe lesion group (18 cases). The degree of coronary stenosis (Gensini score) and the level of ba PWV were analyzed by linear correlation. Using coronary angiography as gold standard, ROC curve was drawn to evaluate the screening value of ba PWV for CHD. Results ba PWV in CHD group was significantly higher than that in control group (P0.05). The corresponding ba PWV values were 1558.43 卤97.26 cm / s, 1698.00 卤100.49 cm / s and 1892.56 卤209.30 cm / s, respectively. There was a positive correlation between the). Ba PWV level and the Gensini score (P 0.05). According to the characteristic of ROC curve, the best cut point of, ba PWV is 1525 cm / s, the Yuedden index is 0.728, the sensitivity is 83.9 and the specificity is 88.9. Using ba PWV1525cm/s as dependent variable of abnormal increase of ba PWV, the results showed that age (OR value 1.112), hypertension (OR value 3.183) and diabetes mellitus (OR value 2.953), HCY (OR value 3.091) were independent risk factors for abnormal increase of ba PWV. The results showed that age (OR value 1.112), hypertension (OR value 3.183) and diabetes mellitus (OR 2.953), HCY (OR value 3.091) were independent risk factors for ba PWV abnormal increase. The results showed that hypertension, diabetes mellitus, ba PWV level, smoking and HCY were independent influencing factors of CHD, their OR values were 6.813 / 5.375 and 78.2475.932 / 4.765, respectively. Conclusion there is a positive correlation between CHD and ba PWV and other risk factors. Ba PWV has a certain value in screening CHD and can predict the severity of coronary artery disease.
