冠心病患者血清糖化血红蛋白A1c型水平与冠状动脉狭窄程度的相关性研究

【摘要】:目的:探讨冠心病(CHD)患者血清糖化血红蛋白A1c型(GHbA1c)水平与冠状动脉狭窄程度的关系及其作用机理。方法:选取126例经冠状动脉造影(CAG)检查确诊为冠心病(CHD)的患者分成2组,其中,冠心病组67例,冠心病并糖尿病(DM)组59例。与此同时,选取除外CHD和DM的人群46例作为对照组。全部研究对象测定血清糖化血红蛋白A1c型(GHbA1c)水平及一般生化指标,并以Gensini积分值作为评估冠状动脉狭窄程度的指标。分析CHD患者血清GHbA1c水平与冠状动脉狭窄程度之间的相关性。结果:CHD组的血清GHbA1c水平(6.16±0.43%)及Gensini积分值(74.5±16.5)均高于对照组(5.52±0.41%,32.5±15.5)(P㩳0.01),CHD合并DM组的血清GHbA1c水平(7.38±0.39%)及Gensini积分值(80.5±13.5)均显著高于CHD组和对照组(P㩳0.01)。采用Spearman秩相关及多因素多元线性回归分析显示,血清GHbA1c水平与Gensini积分值呈正相关(=0.367,P㩳0.01)。结论:CHD患者血清GHbA1c水平与冠状动脉狭窄程度呈正相关。血清GHbA1c水平是影响CHD冠状动脉功能的重要危险因子。
[Abstract]:Objective: to investigate the relationship between serum glycosylated hemoglobin A 1c (GHbA1c) level and coronary artery stenosis in patients with coronary heart disease (CHD). Methods: 126 patients with coronary heart disease (CHD) diagnosed by coronary angiography (CAG) were divided into two groups: coronary heart disease (CHD) group (n = 67), coronary heart disease (CHD) group (n = 59) and diabetes mellitus (DM) group (n = 59). At the same time, 46 subjects excluding CHD and DM were selected as control group. Serum glycosylated hemoglobin A1c (GHbA1c) levels and general biochemical indexes were measured, and the Gensini score was used to evaluate the severity of coronary artery stenosis. To analyze the correlation between serum GHbA1c level and severity of coronary artery stenosis in patients with CHD. Results: the levels of serum GHbA1c (6.16 卤0.43%) and Gensini integral (74.5 卤16.5) in CHD group were significantly higher than those in control group (5.52 卤0.41 卤32.5 卤15.5) (P < 0.01), GHbA1c level (7.38 卤0.39%) and Gensini score (80.5 卤13.5) in DM group were significantly higher than those in CHD group and control group (P0. 01). The Spearman rank correlation and multivariate linear regression analysis showed that the serum GHbA1c level was positively correlated with the Gensini score (= 0.367 P0. 01). Conclusion: the level of serum GHbA1c in patients with CHD is positively correlated with the degree of coronary artery stenosis. Serum GHbA1c level is an important risk factor affecting coronary artery function of CHD.
