老年慢性心力衰竭贫血患者血清肿瘤坏死因子-α、B型钠尿肽及细胞黏附因子-1水平及与心室重构的关系

【摘要】：目的探讨老年慢性心力衰竭贫血患者血清肿瘤坏死因子(TNF)-α、B型钠尿肽(BNP)、细胞黏附因子(ICAM)-1水平与心室重构的关系。方法选取565例老年慢性心力衰竭贫血患者为研究组,另选取586例老年慢性心力衰竭患者为对照组,检测血清TNF-α、BNP、ICAM-1水平,评价两组心功能,计算左心室质量指数(LVMI)、平均室壁应力(MWS)。对比研究组不同贫血程度及不同纽约心脏病协会(NYHA)心功能分级患者血清TNF-α、BNP、ICAM-1水平及LVMI、MWS。结果研究组血清TNF-α、BNP、ICAM-1水平、LVMI、MWS均高于对照组(P0.05)。中度贫血患者TNF-α、BNP、ICAM-1水平、LVMI、MWS高于轻度者(P0.05)。研究组NYHA心功能分级为Ⅳ级患者血清TNF-α、BNP、ICAM-1水平、LVMI、MWS均高于Ⅱ级和Ⅲ级,且Ⅲ级患者均高于Ⅱ级(均P0.05)。结论老年慢性心力衰竭贫血患者血清TNF-α、BNP、ICAM-1水平升高能够促进心室重构的发生和发展。
[Abstract]:Objective To study the relationship between serum tumor necrosis factor (TNF)-1, B-type natriuretic peptide (BNP), cell adhesion factor (ICAM)-1 and ventricular remodeling in elderly patients with chronic heart failure. Methods 565 elderly patients with chronic heart failure were selected as the study group, and the other 586 patients with chronic heart failure were selected as the control group. The levels of TNF-, BNP and ICAM-1 in serum were measured. The left ventricular mass index (LVMI) and the mean wall stress (MWS) were calculated. The levels of TNF-1, BNP, ICAM-1 and LVMI, MWS in serum of patients with different anemia and different New York Heart Association (NYHA) were compared in the study group. Results The levels of TNF-, BNP, ICAM-1, LVMI and MWS in the study group were higher than those in the control group (P0.05). The levels of TNF-1, BNP, ICAM-1, LVMI and MWS in patients with moderate anemia were higher than those of mild anemia (P0.05). The NYHA functional classification of the study group was the level of serum TNF-1, BNP, ICAM-1, LVMI and MWS in the grade IV patients, which were higher than those of grade II and grade III, and all the patients with grade 鈪，
