Artisan虹膜夹持型人工晶状体治疗无晶状体眼的临床观察

【摘要】：目的观察Artisan虹膜夹持型人工晶状体(IOL)治疗无晶状体眼的临床疗效及手术并发症。方法选取接受Artisan虹膜夹持型IOL植入术的无晶状体眼患者13例(13眼),比较患者手术前后裸眼视力、最佳矫正视力和眼压变化,裂隙灯显微镜观察术后角膜水肿、前房闪辉、IOL位置和虹膜情况并行角膜内皮细胞计数。结果术后随访平均2年,裸眼视力及最佳矫正视力超过0.5的有5只眼(38.5%)和6只眼(46.2%),手术前后眼压无明显变化,术后随访角膜无明显水肿,晶状体位置未见明显偏移,前房无明显炎症反应,平均内皮细胞丢失率为10.1%。术中有2例合并虹膜损伤穿孔,有2例发生虹膜出血,但均在术后短期内吸收。术后随访1例IOL脱位,予重新手术固定。随访未发现黄斑囊样水肿。结论 Artisan虹膜夹持型IOL植入术是一种安全有效的治疗无晶状体眼的方法,手术操作相对简单,组织损伤相对较小,并发症少,术后视力提高较为显著。
[Abstract]:Objective to observe the clinical effect and operative complications of Artisan iridoid intraocular lens (IOL) in the treatment of aphakic eyes. Methods Thirteen patients (13 eyes) with aphakic eye underwent Artisan iridoplasty with IOL implantation were selected. The uncorrected visual acuity, best corrected visual acuity and intraocular pressure were compared before and after operation. Corneal edema was observed by slit lamp microscope. The position of IOL and the iris of anterior chamber were counted with corneal endothelial cells. Results the mean postoperative follow-up was 2 years. 5 eyes with uncorrected visual acuity and best corrected visual acuity (BCVA) (38.5%) and 6 eyes (46.2%) had no obvious change in IOP before and after operation. There was no obvious edema in cornea and no obvious deviation of lens position. There was no obvious inflammatory reaction in anterior chamber, and the average loss rate of endothelial cells was 10.1%. There were 2 cases with iris injury and perforation and 2 cases with iris hemorrhage during the operation, but all of them were absorbed in a short period after operation. One case of IOL dislocation was followed up and fixed with reoperation. No macular cystic edema was found at the follow-up. Conclusion Artisan iridoplasty with IOL implantation is a safe and effective method for the treatment of aphakic eyes. The operation is relatively simple, the tissue injury is relatively small, the complications are less, and the postoperative visual acuity is improved significantly.
