不同切口设计的白内障超声乳化术后角膜变化的研究

【摘要】:目的:通过眼科常规检查及角膜地形图仪观察两种不同设计的切口白内障手术前后视力、散光及角膜形态的变化。 方法:收集在我院接受白内障超声乳化吸除联合IOL植人术的患者40例(46眼),随机分为A、B两组。A组作巩膜隧道自闭式切口,B组作透明角膜切口,两组位置均在上方(12:00位),大小为3.0mm,两组核硬度均在Ⅱ-Ⅲ级之间。采用眼科常规检查及角膜地形图仪观察白内障手术前,术后1天、术后1月、术后3月的裸眼视力、散光及角膜前表面的形态变化。 结果:两组手术前的视力比较,差异无统计学意义(PO.05),术后1天A组的裸眼视力好于B组**(P0.05)。术后1天、1月、3月两组的裸眼视力均较术前有所提高*(P0.05)。术后1月、3月的裸眼视力两组比较差别不明显,差异无统计学意义(P0.05)。两组术前的散光比较,差异无统计学意义(PO.05),术后1天、1月、3月A组的散光度数小于B组,两组差异有统计学意义**(P0.05)。而术后三个月和术前比较散光有所减小,两组差异有统计学意义*(P0.05)。角膜地形图两组术后1天上方切口所在的径线较手术前变陡峭,角膜彩色色码图较术前向暖色调变化。随着时间的推移,两组散光均有所减小。切口对应处(12点位)周边部的角膜变得相对平坦,角膜曲率变小,角膜彩色色码图向冷色调变化,其余部分彩色色码图无明显变化。术后1月、3月两组患者的变化不明显。 结论:1.巩膜隧道切口对角膜形态的影响较小、散光较小。2.巩膜隧道切口具有同透明角膜切口相同的优点,术后视力恢复较好、较安全。3.巩膜隧道切口术后早期的视力好于角膜切口组,远期两组的差别不明显。4.选择在角膜最大屈光力的径线上作切口对所在子午线的角膜曲率有松解作用,能在一定程度上矫正部分散光。
