年龄相关性白内障的高阶像差

【摘要】: 目的:观察三种不同类型的年龄相关性白内障的高阶像差,对不同类型年龄相关性白内障患者的高阶像差进行量化分析并比较其变化特点。为进一步改善白内障术后的视觉质量提供理论依据。 方法:回顾性病例分析。选择不同类型的年龄相关性白内障患者109例163眼,年龄44岁~81岁,平均(66.45±9.421)岁,其中女性72例(66.06%)。入选标准为患眼最佳矫正视力"g0.3logMAR(等值球镜度数在-3.0~+3.0D之间),排除有其他任何眼部疾病、眼部手术史及晶状体混浊严重致使Tracey像差仪不能准确测量的患者。根据晶状体混浊分级系统Ⅲ(lens opacities classification systemⅢ, LOCSⅢ)进行白内障分类和分组。其中皮质性59眼,核性62眼,后囊下性42眼,30例30眼透明晶状体作为对照组,使用Tracey像差仪(Tracey wavefront aberrometer, Tracey Technologies Inc., Houston, USA)分别测量6.0mm瞳孔直径下各组的高阶像差。Tracey系统计算和分析共6阶27项Zernike系数和均方根值(root mean square, RMS),比较白内障组和对照组的结果差异,分析并比较不同类型的年龄相关性白内障高阶像差的变化特点。 结果:6.0mm瞳孔直径下,以晶状体皮质混浊为主的皮质性白内障组总高阶像差为(0.701±0.230)μm,高阶主导像差主要以彗差(0.438±0.213)μm为主,其次为三叶草(0.357±0.203)μm,在各高阶像差中,除球差(P=.781)、五叶草(P=.384)、六叶草(P=.390)外,彗差(P=.000)、高级散光(P=.002)、三叶草(P=.002)、四叶草(P=.005)均有统计学意义。以晶状体核混浊为主的核性白内障组总高阶像差为(1.101±0.303)μm,高阶主导像差主要以球差(0.497±0.231)μm为主,其次为彗差(0.466±0.291)μm,在各高阶像差中,球差(P=.000)、彗差(P=.002)、高级散光(P=.003)、三叶草(P=.008)、四叶草(P=.002)、五叶草(P=.006)、六叶草(P=.007)均有统计学意义。以晶状体后囊膜下混浊为主的后囊下性白内障组总高阶像差为(0.645±0.261)μm,高阶主导像差主要以三叶草(0.335±0.142)μm为主,其次为彗差(0.294±0.178)μm,在各高阶像差中,球差(P=.008)、彗差(P=.004)、高级散光(P=.000)、三叶草(P=.000)、四叶草(P=.008)、五叶草(P=.003)、六叶草(P=.001)均有统计学意义。 结论:不同类型的年龄相关性白内障产生不同特点的高阶像差,皮质性白内障主要引起慧差的改变,核性白内障主要使球差增加,后囊下性白内障的三叶草明显增加。白内障患者主视视觉质量的下降与各阶像差不同程度的改变相关。
