两种方法联合评估新疆维吾尔族OSAHS患者的听觉功能

【摘要】：【目的】为了评估阻塞性睡眠呼吸暂停低通气综合征(OSAHS)对新疆维吾尔族听觉功能的影响,探讨其损害部位及特点。【方法】70例维吾尔族成人经多导睡眠监测系统(PSG)检查分为轻、中、重组OSAHS患者,选取35例维吾尔族健康成人为对照组。所有研究对象分别进行畸变产物耳声发射(DPOAE)及听性脑干反应(ABR)检测及分析,通过比较DPOAE的反应幅值及ABR的各波潜伏期和波间期变化评估听觉功能损害情况。【结果】OSAHS患者DPOAE各频率(750~8 000 Hz)的反应幅值较对照组降低,差异有统计学意义(P0.05)。经ABR检测,轻中度OSAHS组与对照相比,波Ⅰ、Ⅲ、Ⅴ潜伏期及Ⅰ-Ⅲ、Ⅰ-Ⅴ波间期的延长,差异有统计学意义(P0.05);重度OSAHS组的各波潜伏期及波间潜伏期均较对照延长,差异有统计学意义(P0.05);轻中度组与重度组OSAHS相比波Ⅰ、Ⅲ潜伏期及Ⅰ-Ⅴ波间期的延长,差异有统计学意义(P0.05)。OSAHS的病情程度与BMI、阻塞部位、鼾声指数、最低血氧饱和度关系密切(P0.05),并且不同病情程度的OSAHS患者的DPOAE检出率、ABR异常率相比较差异有统计学意义(P0.05)。【结论】OSAHS对维吾尔族听觉功能有损害,其中耳蜗主动听觉功能降低,尤以高频听力下降明显,可能合并听觉传导通路中的听神经中枢段及听觉中枢的损害,需联合DPOAE和ABR综合评估筛查听觉损害,尽早发现,及时干预。
