我院静脉药物配置中心高危药品的风险控制

【摘要】：目的:降低医院静脉药物配置中心(PIVAS)中高危药品的调配风险。方法:介绍我院PIVAS对高危药品进行风险控制的主要措施,包括成立高危药品风险控制小组、建立高危药品目录、除配送成品外的PIVAS工作全由药学专业人员承担、高危药品使用流程实现信息化管理环节并强化临床药师的参与等。通过统计近3年PIVAS发生的差错及不良事件评价管理效果并总结经验。结果与结论:2012-2014年,我院PIVAS与高危药品相关的差错发生率低(0.009‰、0.004‰、0.002‰)且逐年减少,由2012年的8件减少为2013年的3件、2014年的2件;其中加强核对、加强新进人员的操作训练、PIVAS人员实现药学专业化及建立高危药品的警示标识对风险控制具有重要作用。
[Abstract]:Objective: to reduce the risk of high risk drug allocation in hospital intravenous drug allocation center. Methods: the main measures of PIVAS risk control for high risk drugs in our hospital were introduced, including the establishment of high risk control group and the establishment of high risk drug catalogue.
