【摘要】：看病难、看病贵是我国城市与农村普遍存在的问题,医疗卫生方面的矛盾近年来异常突出。为了切实解决上述矛盾,维护社会公平正义,我国2009年全面实施深化医药卫生体制综合改革,医药卫生体制综合改革是党中央、国务院的重大决策,是加快医药卫生事业发展、维护人民健康的重大民生工程。在这样的大背景之下,朝阳市作为辽宁省农村面积最大农业人口最多地级市,成为医药卫生体制改革承上启下的一个重要环节,朝阳市政府也承担着完成好改革任务的历史使命。本次医药卫生体制改革旨在坚持公共卫生医疗服务的公益性质,把基本医疗卫生制度作为公共产品向全民提供,摈弃了过度市场化的做法,强化政府在基本医疗卫生制度中的责任。因此落实好政府的财政投入责任,是医改能否落实到位的关键性、经济性的基础。同时,改革牵扯多方利益,卫生服务市场本身存在失灵现象,及利益驱动机制、竞争机制、供求机制、价格机制的局限性和弱点也决定政府还扮演监督者和协调者的角色。本文从公共管理主体的角度出发,分析朝阳市政府在全国医药卫生体制改革的大背景下实施改革所面临的问题和成因,并提出解决的方案。本文作为公共管理案例分析型论文,分为四个章节：第一章绪论,提出研究朝阳市政府实施医药卫生体制改革作用的背景和意义,评述国内外研究的现状,阐述案例论文研究的思路和方法；第二章案例背景,分析朝阳市政府实施医药卫生体制改革的背景及初步进展,分析案例发生的内外部条件及环境,重点介绍案例中产生的问题；第三章案例分析,运用公共管理相关理论分析问题,重点分析其内在原因,并通过对比改革较为成功地区分析差距；第四章提出建议和解决方案,为朝阳市这类财政紧缺资源匮乏的地区提供具体的具有可参考价值的解决方案。
[Abstract]:Medical treatment is difficult and expensive is a common problem in cities and rural areas in China, medical and health contradictions in recent years are extremely prominent. In order to effectively solve the above contradictions, maintain social equity and justice. In 2009, China carried out the comprehensive reform of the medical and health system in an all-round way. Major people's livelihood projects to safeguard the people's health. In such a background Chaoyang as the largest rural area of Liaoning Province the largest agricultural population prefecture-level city has become an important link in the reform of the medical and health system. Chaoyang city government also undertakes the historical mission of completing the reform task. The medical and health system reform aims to adhere to the public nature of public health care services. To provide the basic medical and health system to the whole people as a public product, we have abandoned the excessive marketization and strengthened the responsibility of the government in the basic medical and health system. Therefore, we should carry out the responsibility of the government's financial input. It is the key to the implementation of medical reform and the basis of economy. At the same time, the reform involves various interests, the health service market itself has the phenomenon of failure, and the interest driven mechanism, competition mechanism, supply and demand mechanism. The limitations and weaknesses of the price mechanism also determine that the government also plays the role of supervisor and coordinator. This paper analyzes the problems and causes of the Chaoyang Municipal Government's implementation of the reform under the background of the reform of the national medical and health system, and puts forward some solutions. As a case study of public management, this paper is divided into four chapters: the first chapter is introduction, the background and significance of the research on the role of the Chaoyang Municipal Government in implementing the reform of the medical and health system, and comments on the current research situation at home and abroad. Expound the thought and method of case paper research; The second chapter is case background, analyzes the background and preliminary progress of Chaoyang city government to implement the reform of medical and health system, analyzes the internal and external conditions and environment of the case, and focuses on the problems arising in the case. The third chapter of the case analysis, the use of public management related theory analysis of problems, focusing on its internal reasons, and through the comparative analysis of the gap between the more successful areas of reform; Chapter 4th puts forward suggestions and solutions to provide specific solutions of reference value for the regions with scarce financial resources such as Chaoyang City.
