【摘要】：近年来医患关系已成为一个热门话题,不仅引起大众舆论的关注,而且也引发学术界的讨论。病情告知方式则关系到能否建立起和谐的医护关系。在病情告知方面已有大量的研究探讨如何在维持患者治疗信心和告知的真实性之间取得平衡,而西方的一种六步病情告知法更是引起广泛关注。尽管各研究就如何进行病情告知并未达成一致,但在重症病情告知中使用相应的委婉语是普遍存在的。 文章依据恐惧管理理论分析委婉语如何在死亡突显时降低死亡恐惧感。该理论认为死亡恐惧感会因文化观念及自我价值等结构的强化而消减。因此,本文提出在面对死亡突显等敏感话题时,如进行重症病情告知,可以应用死亡和疾病类委婉语。 本文讨论以下问题：1)恐惧管理理论如何论证医患交流中委婉语的应用；2)死亡和疾病类委婉语如何协助病情告知；3)重症病情告知中死亡和疾病类委婉语的使用策略。 经细致研究文章得出以下结论：1)涉及死亡等敏感话题时,不同文化中都存在使用委婉语的惯例,而在恐惧管理理论看来死亡恐惧感正是通过文化及自我价值等结构来减缓的。因此委婉语常在死亡突显时得以应用,在医学上表现为在重症病情告知中使用死亡和疾病类委婉语,尽量避免使用“死亡”等禁忌语。2)医生在病情告知中使用死亡和疾病类委婉语是一个将敏感话题巧妙地借助诸多语言策略进行间接陈述的过程,以此取得最佳的病情告知效果。3)死亡和疾病类委婉语又可以划分为对诊断术语的委婉语、对重症病情确诊的委婉语,对重症病情评价的委婉语以及对重症病情告知人的委婉语。文章从语义、词汇及语法层面分析此类委婉语的使用策略。 研究分为五个部分。第一章是对文章的总体介绍,涵盖了研究背景、研究目的和意义以及文章研究方法和结构。第二章综述了不同角度的委婉语研究以及委婉语的定义、分类及形成方式。第三章是全文的理论基础,探讨恐惧管理理论对病情告知中死亡和疾病类委婉语的论证。第四章是全文的主体部分,细致分析了委婉语的形成机制,从语义、词汇及语法层面探讨对死亡和疾病类委婉语的再分类。第五章总结全文,包括研究成果、指导意义及本研究的局限之处。
