64层螺旋CT对成人冠状动脉的应用解剖学研究

【摘要】: 目的:①通过64层螺旋CT对成人冠状动脉直径、冠状动脉分叉角度及冠状动脉开口与主动脉根部间距离的测量,探讨冠状动脉与年龄之间的关系及在临床的应用。②分析冠状动脉主干分叉角度与斑块形成的相关性。材料与方法:①选取行64层螺旋CT检查,结果正常者共142例,按不同性别、年龄分成5组。将原始图像在图像后处理工作站上重建出冠状动脉的最大密度投影(MIP)图像和容积重建(VRT)图像。在单层横断面上测量管腔直径,在MIP图像上进行角度及距离测量。②另选取55例临床疑似或确诊患有冠心病病例的64层螺旋CT的冠状动脉造影(CTA)图像,进行冠状动脉左主干分叉角度的测量。所有统计采用SPSS11.0软件进行分析。结果:1.直径:男性①右冠状动脉:31-40年龄与61-70组之间、41-50年龄组与61-70岁组之间、31-40年龄组与71-年龄组之间、41-50年龄组与71-年龄组的直径值之间存在显著统计学差异(P<0.05);②左前降支:31-40年龄组与61-70年龄组及71-年龄组之间,存在显著统计学差异(P<0.05);③左回旋支:31-40年龄组与61-70年龄组之间,31-40年龄组与71-年龄组之间存在显著统计学差异(P<0.05)。女性①右冠状动脉:31-40年龄组与71-年龄组之间、41-50年龄组与71-年龄组的直径值之间存在显著统计学差异(P<0.05);②左前降支:31-40年龄组与71-年龄组之间、51-60年龄组与71-年龄组之间存在显著统计学差异(P<0.05);③左回旋支:31-40年龄组与61-70组之间、41-50年龄组与61-70岁组之间、31-40年龄组与71-年龄组之间、41-50年龄组与71-年龄组的直径值之间存在显著统计学差异(P<0.05)。2.不同年龄组中,冠状动脉与主动脉间,左前降支与左回旋支间的角度及左、右冠状动脉开口至主动脉根部距离之间无显著统计学差异(P>0.05)。3.对于同一年龄组,性别间测量值无显著统计学差异(P>0.05)。4.左前降支近段有斑块患者中,左冠状动脉主干分叉角度>78.5°与左冠状动脉主干分叉角度<78.5°两组之间有显著差异(P<0.05);左回旋支近段有斑块患者中,左冠状动脉主干分叉角度>78.5°与左冠状动脉主干分叉角度<78.5°两组之间无显著差异(P>0.05)。结论:年龄与冠状动脉的直径之间存在着负相关性;左冠状动脉主干分叉角度对冠状动脉粥样硬化斑块的形成起重要的影响作用。
[Abstract]:Objective to measure the diameter of coronary artery, the angle of coronary artery bifurcation and the distance between coronary artery opening and aortic root by 64-slice spiral CT. To explore the relationship between coronary artery and age and to analyze the correlation between coronary artery bifurcation angle and plaque formation. Results 142 cases were normal, according to sex. Age was divided into 5 groups. The original image was reconstructed on the image post processing workstation with maximum density projection (MIP) image and volume reconstruction (VRT) image. The diameter of the lumen was measured on a single cross section. Angle and distance were measured on MIP images. Another 55 patients with suspected or confirmed coronary heart disease were selected for 64-slice spiral CT coronary angiography. The angle of left main coronary artery bifurcation was measured. All statistics were analyzed by SPSS11.0 software. Results: diameter: male 1 right coronary artery:. 31-40 age group and 61-70 group. Between the 41-50 age group and the 61-70 age group, the age group 31-40 and the 71-age group. There was significant difference in diameter between 41-50 age group and 71-age group (P < 0.05). (2) there was a significant difference in the left anterior descending branch between the 31-40 age group and the 61-70 age group and the 71-age group (P < 0. 05); 3The left circumflex branch: 31-40 age group and 61-70 age group. There was significant statistical difference between 31-40 age group and 71-age group (P < 0.05). Female 1 right coronary artery: 31-40 age group and 71-age group. There was significant difference in diameter between 41-50 age group and 71-age group (P < 0.05). (2) there was significant statistical difference between 71-60 and 71-60 age groups in the left anterior descending branch (P < 0.05). 3The left circumflex branch was between the age group of 31-40 and the group of 61-70, and the age group of 41-50 and the age group of 61-70 years, between the age group of 31-40 and the age group of 71-70. There was significant difference in diameter between 41-50 age group and 71-age group (P < 0.05). There was significant difference between coronary artery and aorta in different age groups. There was no significant difference in the angle between the left anterior descending branch and the left circumflex branch and the distance between the opening of the left and right coronary artery and the root of the aorta (P > 0.05. 3) for the same age group. There was no significant statistical difference between sexes (P > 0.05). 4. In patients with plaque in proximal segment of left anterior descending branch. There was significant difference between the two groups in the angle of main coronary artery bifurcation > 78.5 掳and the angle of left main coronary artery bifurcation < 78.5 掳(P < 0.05). In patients with plaque in the proximal segment of the left circumflex branch. There was no significant difference between the left coronary artery shunt angle > 78.5 掳and the left coronary artery bifurcation angle < 78.5 掳(P > 0.05). Conclusion: there is a negative correlation between age and coronary artery diameter. The angle of bifurcation of left coronary artery plays an important role in the formation of coronary atherosclerotic plaque.
