眼眶球后断层解剖及三维可视化研究

【摘要】: 研究目的:用断层解剖学方法研究眼眶球后结构的形态以及内部重要结构的相互关系,并进行三维可视化研究,阐明球后结构的空间位置关系。为临床治疗提供解剖学依据。 材料与方法:用石蜡包埋法对1例离体眶尖标本进行处理,采用冠状位连续切片,层厚为10um,获得眼眶球后结构解剖断面图像数据集。并将同一头颅标本行T1WI、T2WI序列多方位MRI检查以及高分辨CT横断、直接冠状位扫描,观察各层面的构成形态以及相互关系。再将断面结构数据图像经Photoshop8.0软件精确对位以及配准后,应用3D-DOCTOR软件在计算机上分别对眼球、眼外肌、视神经、眼动、静脉及其分支等结构进行三维重建并立体显示,获得直观的三维立体构型。应用该软件自带工具测量球后数据并与临床肿瘤资料相结合,为临床诊治提供依据。 结果: 1.明确眼眶球后结构的连续断面结构数据以及眼外肌、神经、血管的关系,完成眼眶球后部分的数据测量和形态观察。 2.将断面图像与相应的CT、MRI图像进行对照研究,完成眼眶球后结构断面影像解剖结构的对照观察。 3.获得完整的球后结构的三维立体模型,明确球后神经、血管以及眼外肌的空间位置关系,用于指导临床。 结论:本试验通过对眼眶球后结构的断层解剖学研究,着重探讨眼眶球后内部结构的大小、形态学表现,对影像学研究提供断层解剖学依据,同时为球后病变的诊断与鉴别诊断提供依据。此外获得的球后结构的三维立体模型,为计算机辅助虚拟手术和数字化模拟教学提供了良好的平台。
[Abstract]:Objective: to study the relationship between the shape of the posterior orbital structure and the internal important structure by means of sectional anatomy, and to make a three-dimensional visualization study to clarify the spatial and positional relationship of the retroglobular structure. To provide anatomic basis for clinical treatment. Materials and methods: a case of isolated orbital apex was treated with paraffin embedded method. The image data set of anatomical section of the orbital retroglobus structure was obtained by coronal serial section with the thickness of 10 umps. The same head specimen was examined by multidirectional MRI and high resolution CT transection and coronal scan. The morphology of each plane and the relationship between them were observed. Then the cross-sectional structure data were accurately aligned and registered by Photoshop8.0 software, and 3D reconstruction and stereoscopic display of the structures of eyeball, extraocular muscle, optic nerve, eye movement, vein and its branches were carried out using 3D-DOCTOR software on the computer, respectively. An intuitive three-dimensional configuration is obtained. The software has been used to measure the back ball data and combined with the clinical tumor data to provide the basis for clinical diagnosis and treatment. Results: 1. The data of the continuous cross-sectional structure and the relationship among the extraocular muscles, nerves and blood vessels were determined, and the data of the posterior part of the orbital ball were measured and observed. 2. The cross-sectional images were compared with the corresponding CT MRI images to observe the anatomical structure of the orbital retroglobus structure. 3. A complete three-dimensional model of the retrobulbar structure was obtained, and the spatial relationship of the retroglobular nerve, blood vessels and the extraocular muscle was determined to guide the clinical practice. Conclusion: this study focuses on the size and morphology of the internal structure of the orbital ball through the sectional anatomical study of the orbital retroglobus structure, and provides the sectional anatomical basis for the imaging study. It also provides basis for diagnosis and differential diagnosis of Retrobulbar lesions. In addition, the three-dimensional model of the back-ball structure is obtained, which provides a good platform for computer-aided virtual surgery and digital simulation teaching.
