犬胸主动脉内国产分体式覆膜支架置入术的实验研究

【摘要】:目的: 评价犬胸主动脉内国产分体式覆膜支架置入术的技术可行性与生物相容性。 材料和方法: 成年杂种犬12只,随机分为3组,每组4只。右侧股动脉切开,将国产分体式主动脉覆膜支架置于犬胸主动脉内,且使外层移植物的近端至少接近左锁骨下动脉开口的远端,观察支架释放的技术可行性。按照实验分组分别于术后4周、8周、12周血管造影复查分体式覆膜支架在犬胸主动脉内情况,后处死实验犬获取标本,大体观察及HE染色显微镜下观察支架与血管壁贴合情况、支架内表面内膜化情况、血管壁病理变化等。 结果: 10只犬成功经右侧股动脉将国产分体式主动脉覆膜支架置于犬胸主动脉内,无需降压,释放顺利精确,无移位变形;1只犬因内支架输送系统进入时右侧股动脉撕裂出血而死亡,1只犬因外层支架移植物向前移位致升主动脉破裂立即死亡。术后4周、8周、12周血管造影复查示支架位置及开放良好,无移位、变形、裂断、再狭窄发生。两端裸支架4周时完全内膜化,支架移植物的内表面4周起出现内膜化,随着时间的推移,内膜化逐渐明显,12周时内膜基本覆盖支架移植物。 结论: 介入经犬股动脉置入国产分体式主动脉覆膜支架于胸主动脉内,技术上是可行的,并具有良好的生物相容性。
[Abstract]:Objective: to evaluate the technical feasibility and biocompatibility of canine thoracic aortic stent implantation. Materials and methods: 12 adult mongrel dogs were randomly divided into 3 groups with 4 dogs in each group. The right femoral artery was incised and the domestic split aortic stent was placed in the thoracic aorta of the dog and the proximal end of the outer graft was at least close to the distal end of the left subclavian artery opening. The technical feasibility of stent release was observed. After 4 weeks, 8 weeks and 12 weeks after operation, the segmented stent was performed in the thoracic aorta of dogs, and then the dogs were killed to obtain the specimens. Gross observation and HE staining microscope were used to observe the adhesion of stents to vascular wall, the intimation of stent surface and the pathological changes of vascular wall. Results: 10 dogs were successfully placed into the thoracic aorta via the right femoral artery, without any hypotension, accurate release and no displacement. One dog died after the right femoral artery laceration and one dog died of ascending aorta rupture due to the forward displacement of the outer stent graft. Angiographic examination at 4, 8 and 12 weeks postoperatively showed that the stent was well located and open, without displacement, deformation, fracture and restenosis. The endarteal endomativization of the bare stents at both ends was complete at 4 weeks and the inner surface of the stents was intimated at 4 weeks. With the passage of time the intimation became more and more obvious and the intima covered the stents basically at 12 weeks. Conclusion: it is technically feasible and has good biocompatibility for interventional placement of domestic split aortic stent into thoracic aorta through femoral artery.
