2型糖尿病合并高血压患者血清同型半胱氨酸水平的变化及其原因分析

【摘要】:目的:探讨2型糖尿病合并高血压患者同型半胱氨酸水平的变化及其可能的原因。方法:以2016年1月至2016年12月在我院住院的2型糖尿病合并高血压患者85例为糖尿病并高血压组,无高血压的2型糖尿病患者73例为单纯糖尿病组,同期体检中心健康体检者32例为正常对照组。所有入选对象均符合诊断标准与排除标准。收集所有研究对象的临床资料,测定其一般生化指标,并测定血清同型半胱氨酸(Hcy)、叶酸、维生素B12(VitB12)、空腹胰岛素(FINS)水平。再以血清Hcy水平大于等于15μmol/L为界,将糖尿病并高血压组分为同型半胱氨酸水平升高组(Hcy升高组)和同型半胱氨酸水平正常组(Hcy正常组)。结果:糖尿病并高血压组的Hcy水平高于单纯糖尿病组及正常对照组,叶酸水平低于单纯糖尿病组及正常对照组,差异具有统计学意义(P0.05)。Hcy升高组的叶酸水平明显低于Hcy正常组,差异有统计学意义(P0.05)。相关分析显示2型糖尿病合并高血压患者血清Hcy水平与血肌酐(Cr)、尿素氮(BUN)正相关,与叶酸负相关,差异均有统计学意义(P0.05),与年龄、身高、体重、BMI、收缩压、舒张压、空腹血糖、餐后2小时血糖、糖化血红蛋白、ALT、AST、UA、TG、VitB12、HOMA-IR均无线性相关关系(P0.05)。结论:2型糖尿病合并高血压患者血清Hcy水平升高,这可能是由于叶酸水平降低所致。
[Abstract]:Objective: to investigate the changes of homocysteine level in patients with type 2 diabetes mellitus complicated with hypertension and its possible causes. Methods: from January 2016 to December 2016, 85 patients with type 2 diabetes mellitus complicated with hypertension were enrolled in our hospital. Diabetes with hypertension, Seventy-three patients with type 2 diabetes without hypertension were treated with simple diabetes mellitus, while 32 patients with healthy physical examination in the same period were normal control group. All the subjects were in accordance with the diagnostic criteria and exclusion criteria. The clinical data of all the subjects were collected. The serum levels of homocysteine, folic acid, vitamin B12 and fasting insulin were determined. The serum Hcy level was greater than or equal to 15 渭 mol/L. The patients with diabetes mellitus complicated with hypertension were divided into two groups: hyperhomocysteine (Hcy) and homocysteine (Hcy). Results: the level of Hcy in diabetic patients with hypertension was higher than that in diabetic patients and normal controls. The level of folic acid was lower than that of simple diabetes mellitus group and normal control group, and the level of folic acid in the group with elevated P0.05 + Hcy was significantly lower than that in the normal group of Hcy. The correlation analysis showed that the serum Hcy level was positively correlated with creatinine, bun and folic acid in patients with type 2 diabetes mellitus complicated with hypertension, and negatively correlated with folic acid, and was significantly correlated with age, height, body weight and systolic blood pressure. Diastolic blood pressure, fasting blood glucose, 2 hours postprandial blood glucose, and glycosylated hemoglobin TGVitB12Homa Homa IR had no linear correlation. Conclusion the serum Hcy level in patients with type 2 diabetes mellitus with hypertension is increased, which may be due to the decrease of folic acid level.
