血府逐瘀汤联合西药治疗冠心病心绞痛的临床研究

【摘要】：目的:观察血府逐瘀汤联合西药治疗冠心病稳定型劳累性心绞痛(中医辨证为胸痹心脉瘀阻型)患者,与西医常规治疗在心绞痛症状改善和中医症候变化等方面的改善情况。方法:选择自2014年12月至2017年1月期间就诊于延吉市中医医院门诊及住院患者总计60例。病情既属于冠心病心绞痛的西医确诊标准,又符合中医辨证胸痹心血瘀阻型。通过随机原则将60例患者分为西医常规治疗对照组(30例)和中西医结合治疗组(30例),两组均给予对症治疗,治疗组采用中西医结合即加减应用血府逐瘀汤。每14天为1个疗程,3个疗程后进行评估疗效。将患者中医症候评分、心绞痛症状、心电图、硝酸甘油用量作为疗效评估的主要手段,对比并分析其改变。结果:两组患者在心绞痛症状和中医症候方面比较治疗前均有改善,说明两组患者在减少心绞痛发作次数、心绞痛发作的持续时间与心绞痛发作的严重程度治疗有效,且中西医结合改善疗效更显著。在通过心电图评价治疗效果方面,两组患者对比试验前均有正向改变,皆在心电图表现上减轻心肌缺血,且中西医结合治疗组与对照组相比效果更显著。两组患者硝酸甘油用量与治疗前比较有显著差异,且中西医结合治疗组好于对照组。两组患者在试验期间生命体征,实验室检查均无异常,无胃肠道反应、心血管不良反应、过敏反应。结论:血府逐瘀汤加减治疗冠心病心绞痛(中医辨证为胸痹心脉瘀阻型)与西医常规治疗,中西医结合的试验结果更佳,可减少心绞痛发作次数、严重程度以及心绞痛发作时间,减少硝酸甘油用量从而提高患者的生命质量,并且安全性较高。
[Abstract]:Objective: to observe the therapeutic effect of Xuefu Zhuyu decoction combined with western medicine on stable angina pectoris of coronary heart disease (TCM syndrome differentiation is chest obstruction and heart stasis type), and the improvement of angina pectoris symptoms and changes of TCM symptoms with routine treatment of western medicine.Methods: a total of 60 outpatients and inpatients were selected from December 2014 to January 2017 in Yanji traditional Chinese Medicine Hospital.The disease is not only the diagnostic standard of coronary heart disease angina pectoris, but also consistent with TCM syndrome differentiation.60 patients were randomly divided into two groups: control group (n = 30) and integrated Chinese and western medicine group (n = 30). Both groups were given symptomatic treatment, and the treatment group was treated with Xuefu Zhuyu decoction by combination of traditional Chinese and western medicine.Every 14 days as a course of treatment, 3 courses of treatment were evaluated.TCM symptom score, angina pectoris symptom, electrocardiogram and nitroglycerin dosage were compared and analyzed.Results: the symptoms of angina pectoris and symptoms of traditional Chinese medicine were improved in both groups, indicating that the two groups were effective in reducing the number of angina pectoris attack, the duration of angina pectoris attack and the severity of angina pectoris attack.And integrated Chinese and western medicine to improve the efficacy is more significant.In the evaluation of therapeutic effect by electrocardiogram, the two groups had positive changes before the contrast test, both of them alleviated myocardial ischemia in electrocardiogram, and the effect of integrated Chinese and western medicine treatment group was more significant than that of the control group.The dosage of nitroglycerin in the two groups was significantly different from that before treatment, and the combination of traditional Chinese and western medicine was better than that in the control group.There were no abnormal vital signs and laboratory findings, no gastrointestinal reactions, adverse cardiovascular reactions and allergic reactions in both groups.Conclusion: Xuefu Zhuyu decoction can be used to treat angina pectoris of coronary heart disease (syndrome differentiation of traditional Chinese medicine is the type of stagnation of heart and heart of heart) and routine treatment of western medicine. The test result of combination of traditional Chinese and western medicine is better and can reduce the frequency of angina pectoris attack.Severity and angina attack time, reducing nitroglycerin use to improve the quality of life, and high safety.
