全科医学健康管理在社区高血压患者中的应用现状分析

【摘要】：目前我国高血压患病率及并发症增长迅速,已成为中国人群心脑血管疾病最重要的危险因素之一。高血压的防治仅依靠医院门诊是不够的,必须重视并加强社区人群预防、筛查和管理,改革和建立能提供长期稳定社区人群的服务体系。全科医学团队干预是一种新型的、有效的社区干预模式,其高血压社区健康管理模式在提高知晓率、生活方式改善、血压达标率、靶器官保护等方面起着积极的作用。本文就全科医学团队式健康管理在高血压患者中的应用现状进行综述。
[Abstract]:At present, the prevalence and complications of hypertension are increasing rapidly in China, which has become one of the most important risk factors for cardiovascular and cerebrovascular diseases in Chinese population. It is not enough to rely on hospital outpatient service for the prevention and treatment of hypertension. It is necessary to pay attention to and strengthen the prevention, screening and management of community population, and reform and establish a service system that can provide long-term stable community population. General practice team intervention is a new and effective community intervention model. Its hypertension community health management mode plays an active role in improving awareness rate, lifestyle improvement, blood pressure compliance rate, target organ protection and so on. This article reviews the application of group health management in patients with hypertension.
