老年慢性心力衰竭患者可溶性致癌抑制因子2的水平及临床意义

【摘要】:目的探讨血清可溶性致癌抑制因子2(s ST2)与纽约心脏病协会(NYHA)心功能分级的相关性及对老年慢性心力衰竭的诊断价值。方法选取年龄60岁、有明确诊断为慢性心力衰竭的老年患者108例为研究组,老年健康体检者53例作为对照组。入组后抽血测定N端脑钠肽前体(NT-pro BNP)、s ST2、心肌肌钙蛋白(c Tn)I2、肌红蛋白(MYO)、肌酸激酶同工酶(CK-MB),48 h内完成超声心动图检查。结果 s ST2在心力衰竭组较对照组明显升高〔(31.2±15.3)vs(12.5±9.8)μg/L,P0.05〕,不同心功能级别之间s ST2浓度差异显著(F=48.63,P0.01);研究组中s ST2和NT-pro BNP明显正相关(r=0.631,P0.05);s ST2和左心室射血分数(LVEF)明显负相关(r=-0.682,P0.05)。结论慢性心力衰竭患者中s ST2明显升高,且随NYHA分级升高有上升趋势,是评价老年心功能较为敏感的实验室指标。
