【摘要】: 本研究拟通过对10侧新鲜成人下肢标本的显微解剖,对股深动脉第一穿动脉进行系统观测,以此支及伴行静脉为血管蒂设计游离皮瓣,为四肢软组织缺损的修复提供一种新的手术方法。 研究发现,股深动脉第一穿动脉穿出大收肌后发出升支,第一穿动脉主干向远端移行为降支。降支于距大转子水平226.84mm±51.09mm处发出股后远端皮支,后者于距大转子水平264.12mm±20.30mm处穿出深筋膜,穿出深筋膜时血管外径为0.64mm±0.09mm。股后远端皮支自穿出深筋膜到分成上、下行支的长度为36.09mm±6.59mm。股深动脉第一穿动脉股后近端皮支的起源变异较多。 股深动脉第一穿动脉股后远端皮支有两条伴行静脉。二者汇入第一穿动脉降支伴行静脉,后者汇入第一穿动脉伴行静脉。 根据实验所得解剖数据,设计以股深动脉第一穿动脉为血管蒂的游离皮瓣:轴心点位于股后中线距大转子水平27cm及10cm处,轴心线为股后股骨髁间连线中点上方5cm向臀下垂直连线处,切取范围为20cm×11cm。本课题设计的皮瓣为临床修复软组织缺损提供了一种新的手术方法,具有临床应用价值。
[Abstract]:In this study, the first perforating artery of deep femoral artery was systematically observed by microdissection of 10 fresh adult lower limb specimens, which was used to design free flap with venous pedicle. To provide a new surgical method for the repair of soft tissue defects in extremities. The study found that the first perforating artery of the deep femoral artery sent out the ascending branch after perforating the adductor Magnus. The first perforating artery moved to the distal end and the descending branch was located at 226.84 mm 卤51.09 mm from the trochanter level. The latter perforated the deep fascia at a distance of 264.12mm 卤20.30mm from the level of the greater trochanter. The external diameter of the vessel was 0.64 mm 卤0.09 mm when the deep fascia was perforated. The cutaneous branch of the distal end of the femur perforated the deep fascia to the upper part of the fascia. The length of the descending branch was 36.09 mm 卤6.59 mm. the origin of the proximal cutaneous branch of the deep femoral artery after the first perforating artery was more variable. There are two accompanying veins in the cutaneous branch of the posterior distal end of the deep femoral artery into the descending branch of the first perforating artery, and the latter into the first perforating artery with the vein. Based on the anatomical data obtained from the experiment, a free flap pedicled with the first perforating artery of deep femoral artery was designed. The axial point was located at 27cm and 10cm from the trochanter level. The axis line was 5 cm above the midpoint of the intercondylar line of the posterior femur and perpendicular to the bottom of the hip. The flap designed in this paper provides a new surgical method for clinical repair of soft tissue defects and has clinical application value.
