肾动脉多排CTA的相关解剖学研究

【摘要】:目的: 通过多排CTA研究肾动脉及其血管变异相关解剖学数据,目的是为更好的清晰肾动脉的解剖结构,和复杂的肾动脉解剖变异分类、分型。为临床上相关的腹腔镜手术,介入治疗,肾脏移植提供所需的解剖学资料。 材料与方法: 选择2008年11月~2010年01月疑有肾脏病变而行64层螺旋CT增强检查的512例患者影像资料,其中男356例,女156例,年龄18~82岁,平均52.5岁,使用东芝Aquilion64层螺旋CT机,扫描范围为膈肌至髂嵴。观察双侧肾动脉内径、长度、肾门处动脉上缘距肾上极的距离并定位开口位置。观察双侧副肾动脉内径,长度,并计算出现率。观察变异肾动脉类型并统计各类型出现次数。 结果: 1.左肾的长径、宽径、及厚度均大于右侧。左侧肾脏长径103.2±5.0mm,宽径51.2±5.0mm,厚度49.1±6.2mm;右侧肾脏长径99.9±4.0mm,宽径50.4±5.4mm,厚度46.5±4.0mm。 2.左侧肾动脉内径大于右侧,长度小于右侧。80.4%右侧肾动脉开口位置高于左侧,,平均高出5.07±7.43mm。 左侧肾动脉内径5.0±0.7mm,长度44.4±4.7mm,肾门处动脉上缘距肾上极的距离43.6±8.9mm。右侧肾动脉内径5.4±0.7mm,长度53.4±6.0mm,肾门处动脉上缘距肾上极的距离40.6±6.5mm。 3.副肾动脉开口:由肾上端入肾者32支,占45.7%;由肾下端入肾者25支,占35.7%;由肾门周围入肾者13例,占18.6%。左侧副肾动脉内径2.64±0.6mm,长度44.78±3.6mm,出现率42.40%。右侧副肾动脉内径2.58±0.4mm,长度46.76±4.2mm,出现率41.15%。 肾动脉变异类型统计结果Ⅰ类变异A:21,B:10,C:14,D:1,E:2。Ⅱ类变异A:3,B:1,C:0,D:0,E:4。 结论 1.多排螺旋CTA可直观地、准确地显示肾动脉及其变异的数目。 2.肾脏动脉的变异水平较大,在术前行肾脏CTA检查可明确肾脏动脉的变异情况,从而避免术中损伤。
[Abstract]:Objective: The anatomical data related to the variation of renal artery and its vessels were studied by multi-row CTA in order to better understand the anatomical structure of renal artery and to classify the complex anatomic variation of renal artery. Type. Provide anatomical data for clinical related laparoscopic surgery, interventional therapy and kidney transplantation. Materials and methods: From November 2008 to January 2010, a total of 512 patients with suspected renal lesions were examined by 64-slice spiral CT, including 356 males and 156 females. The mean age was 52. 5 years old. Toshiba Aquilion64 slice spiral CT was used. The scanning range was from diaphragm to iliac crest. The diameter and length of bilateral renal arteries were observed. The distance from the superior edge of the renal artery to the upper pole of the renal artery and the location of the opening were observed and the internal diameter and length of the bilateral accessory renal artery were observed and the occurrence rate was calculated. The type of the variant renal artery was observed and the occurrence times of each type were counted. Results: 1. The length, width and thickness of the left kidney were larger than those of the right. The left kidney was 103.2 卤5.0mm in length, 51.2 卤5.0mm in width and 49.1 卤6.2mm in thickness. The right kidney was 99.9 卤4.0 mm in length, 50.4 卤5.4 mm in width and 46.5 卤4.0 mm in thickness. 2. The diameter of left renal artery was larger than that of right, and the length of right renal artery was smaller than that of right. 80.4% the position of right renal artery opening was higher than that of left side, which was 5.07 卤7.43 mm higher than that of left side. The left renal artery was 5.0 卤0.7 mm in diameter and 44.4 卤4.7 mm in length. The distance from the superior edge of the renal artery to the upper pole of the renal artery was 43.6 卤8.9 mm, the diameter of the right renal artery was 5.4 卤0.7 mm and the length of the right renal artery was 53.4 卤6.0 mm. The distance from the superior edge of the renal artery to the upper pole of the kidney was 40.6 卤6.5 mm. 3. Accessory renal artery orifice: 32 of them entered the kidney from the upper end of the kidney, accounting for 45.7%; 25 of them entered the kidney from the lower end of the kidney, accounting for 35.7; The left accessory renal artery was 2.64 卤0.6mm in diameter and 44.78 卤3.6mm in length. The diameter and length of the right accessory renal artery were 2.58 卤0.4mm and 46.76 卤4.2mm, respectively.
