正常听力儿童蜗神经直径及其与年龄相关性的MR研究

【摘要】:目的: 测量不同年龄段正常听力儿童蜗神经的直径,同时探讨其与年龄的相关性。 资料与方法: 搜集2011年11月至2014年3月因非听力原因于山东省医学影像研究所行颅脑MR检查的儿童156例,按年龄分为13组,即1个月-≤6个月、7个月-≤1岁以及2岁-12岁每间隔一岁为一组,每组随机连续选取12例,其中男、女各6例。所有受试者均在常规颅脑MR检查基础上增加内耳三维稳态进动快速成像(the fast imaging employed steady-state acquisition, FIESTA)序列扫描。全部受试者家属均签署知情同意书,并获得本单位伦理委员会批准。将听力筛查合格、颅脑及内耳MRI显示不存在内耳或内听道结构异常及颅内占位性病变、无神经系统感染及耳毒性药物应用史、无个人或家族耳病史、身高、体重等发育正常者纳入入组标准,根据入组标准最终纳入312只内耳作为研究对象,男78例,女78例;左耳156只,右耳156只;年龄1个月-12岁,平均6.2±5.8岁。采用GE3.0T超导MR扫描仪(signa HDxt; GE medical systems,Milwaukee, Wis),8通道相控阵头线圈为接收线圈,成像序列包括常规颅脑冠状位T2WI FSE序列和内耳3D-Fiesta序列扫描。在双侧蜗神经入耳蜗处,选择清晰显示蜗神经的横断面原始图像放大5倍后分别测量其直径;在斜矢状位图像上选取双侧内听道中点、内听道底,测量蜗神经的上下径及前后径,计算上下径及前后径的平均值作为蜗神经的直径。采用双盲法由两位高年资MRI诊断医师独立进行测量,取两人的平均值作为最终测量结果。对两名阅片者的测量结果行组内相关系数(intraclass correlation coefficient,ICC)检验;对不同性别、侧别间蜗神经直径行t检验;Pearson相关分析用于比较年龄和彼此参数间的相关性;单因素及多因素方差分析分别用于评价蜗神经直径在不同测量点、及其随年龄增长的变化,P0.05有统计学意义。 结果: 内耳的3D-Fiesta横轴位图像上可以看到入耳蜗处的蜗神经,显示为线状低信号通入耳蜗;斜矢状位图像上面神经、蜗神经、前庭上神经及前庭下神经在图像上能清晰显示,显示为类圆形、椭圆形及逗号形低信号。2名阅片者对156例研究对象312耳的蜗神经在不同测量点的测量结果和ICC值显示,内听道中点及内听道底处的蜗神经直径比入耳蜗处蜗神经的直径明显增大,正常值分别为1.12±0.08mm、1.05±0.06mm、0.87±0.14mm(P0.05)。从156名双耳正常听力的研究对象中分析,各年龄组儿童蜗神经在入耳蜗处、内听道中点及内听道底处的直径在侧别、性别间无明显差异,P0.05。入耳蜗处、内听道中点及内听道底处的儿童蜗神经直径在年龄组间无明显相关性,P0.05。 结论: 听力正常的儿童蜗神经直径在内听道中点及内听道底均大于入耳蜗处;且正常值不随年龄的变化而改变。初步建立的儿童蜗神经正常值范围有助于先天性及获得性感音性耳聋(Sensorineural Hearing Loss, SNHL)患者CI术前蜗神经的评估。
[Abstract]:Objective:
The diameter of the cochlear nerve in children with normal hearing in different ages was measured, and the correlation with age was also discussed.
Information and methods:
From November 2011 to March 2014 in Shandong Province, the reason for non hearing Medical Imaging Research Institute for brain MR examination of 156 cases of children were divided into 13 groups, namely 1 months less than 6 months, 7 months and less than 1 years and 2 years old -12 years old every year as a group, each group were continuously select 12 cases, male, female 6 cases. All of the subjects families signed informed consent, and approved the ethics committee. Hearing screening qualified, brain and inner ear MRI shows that there is no inner or internal auditory canal structural abnormalities and intracranial lesions, no nervous system infection and ototoxic medication history, no personal or family history of the disease, height, weight and other normal development into the group The standard, according to the inclusion criteria into the final 312 only the inner ear as the object of study, male 78 cases, female 78 cases; 156 left ear, right ear 156; aged 1 months -12 years old, average 6.2 + 5.8 years. By using double blind method by two senior MRI radiologists independently measured, two of the average as the final measurement The amount of results. Measurement results of two readers for the intraclass correlation coefficient (intraclass correlation, coefficient, ICC) for inspection; gender, side between the cochlear nerve diameter t test; Pearson correlation analysis was used to compare the correlation between age and other parameters; univariate and multivariate analysis of variance were used for evaluation.
