安徽省低年级医科大学生自杀意念的心理、社会影响因素及干预效果评价

【摘要】: [目的]了解低年级大学生自杀意念的影响因素,评价以学校为基础的三级高危个体干预和识别系统对降低低年级大学生自杀意念率的作用。 [方法]选择安徽省三所省属医学院校(安徽医科大学、蚌埠医学院和皖南医学院)的10344名一、二年级在校大学生作为研究对象,探讨自杀意念的影响因素。将安徽医科大学作为干预组,蚌埠医学院和皖南医学院同时作为对照组评价以学校为基础的三级高危个体干预和识别系统对降低低年级大学生自杀意念率的效果。干预前后用相同的自制的大学生心理健康状况与相关生活经历调查表(包括自杀态度量表、自杀可接受性量表、负性生活事件量表、家庭功能量表(APGAR)、生活质量量表、Beck焦虑量表(BAI)、Beck自杀意念量表(BSS)、冲动性量表(IMP)、攻击性量表(AGG)、Beck抑郁量表(BDI)等)对目标人群进行调查。 [结果]低年级医科大学生自杀意念率为48.1%(4974/10344);女生自杀意念率52.5%(2921/5564),男生自杀意念率42.9%(2053/4780),女生高于男生,差异有统计学意义(P0.05)。 基线调查自杀意念的多因素分析结果显示年级、性别、自杀的自我控制性、对自杀的歧视、对自杀的正性态度、自杀的可接受性、生活质量、焦虑、攻击性、抑郁、社会支持、负性生活事件和家庭功能是自杀意念的影响因素。女生与年级、对自杀的歧视、对自杀的正性态度、自杀的社会重要性、自杀的可接受性、生活质量、焦虑、攻击性、抑郁、社会支持和负性生活事件有关;男生与恋爱现状、对自杀的歧视、对自杀的正性态度、自杀的可接受性、生活质量、焦虑、攻击性、抑郁、社会支持和负性生活事件有关。 随访分析显示,以基线自杀意念程度和随访中影响自杀意念的因素作为校正变量分析显示干预措施——以学校为基础的三级高危个体识别和支持系统对降低自杀意念有一定的效果。 [结论]低年级医科大学生自杀意念是多种心理、社会因素综合作用的结果,以学校为基础的三级高危个体识别和支持系统可以降低大学生的自杀意念率,应该更加完善已建立的干预系统,切实做好大学生自杀预防工作,并探索更好的防御机制。
[Abstract]:Objective To understand the factors influencing the suicidal thoughts of low - grade college students , and to evaluate the role of the three - level high - risk individual intervention and identification system based on school to reduce the suicide rate of low - grade college students .



In this paper , the effect of self - made mental health status of Anhui Medical University ( Anhui Medical University , Bengbu Medical College and Southern Anhui Medical College ) on the rate of suicidal thoughts of low grade college students was investigated . The effect of intervention and identification system on suicidal thoughts was evaluated by using the same self - made college students as intervention group , Bengbu Medical College and Southern Anhui Medical College as control group .



Results The suicidal rate of suicide in junior college students was 48.1 % ( 4974 / 10344 ) , the suicide rate of female students was 52.5 % ( 2921 / 5564 ) , the suicide rate of male students was 42.9 % ( 2053 / 4780 ) , the female students were higher than boys , the difference was statistically significant ( P0.05 ) .



The multi - factor analysis of suicidal thoughts from baseline revealed that grade , gender , suicide self - control , suicide - based discrimination , positive attitude towards suicide , suicide acceptability , life quality , anxiety , aggression , depression , social support , negative life events , and family function were related to suicide . Male - to - fall , suicide , life quality , anxiety , aggression , depression , social support and negative sexual life events were related ; male - to - love status , suicide - related discrimination , positive attitudes towards suicide , suicide acceptability , quality of life , anxiety , aggression , depression , social support and negative life events .



The follow - up analysis showed that the factors influencing the suicidal thoughts in the baseline suicide degree and the follow - up were used as correction variables . The three - level high - risk individual identification and support system based on school had a certain effect on reducing suicidal thoughts .



Conclusion The suicidal thoughts of college students in lower grades are the result of various psychological and social factors , and the three - level high - risk individual identification and support system based on school can reduce the suicide rate of college students , and the established intervention system should be perfected , the suicide prevention work of college students should be improved , and better defense mechanism is explored .
