碳青霉烯耐药鲍曼不动杆菌对多粘菌素B耐药机制研究

【摘要】: 目的: 检测碳青霉烯耐药鲍曼不动杆菌中常见的β-内酰胺酶基因。体外诱导鲍曼不动杆菌对多粘菌素B耐药,研究其耐药机制。 方法: 收集浙江省8家医院和河南省1家医院的98株碳青霉烯耐药鲍曼不动杆菌。采用琼脂稀释法和E-test法测定14种抗菌药物的最小抑菌浓度。脉冲场凝胶电泳(PFGE)分析菌株的同源性。PCR扩增常见的碳青霉烯酶基因。体外诱导鲍曼不动杆菌对多粘菌素B耐药,探讨不动杆菌对多粘菌素B的耐药机制。 结果: 98株鲍曼不动杆菌对包括碳青霉烯类在内的多种抗生素耐药,对多粘菌素B全部敏感。替加环素、阿米卡星和环丙沙星的耐药率分别为88.8%、94.4%和97.7%。其余10种抗生素的耐药率均100%。根据耐药表型选取的48株菌株的PFGE条带相同或相似,为同一克隆株或密切相关菌株。98株细菌中,94株产OXA-23型碳青霉烯酶,41株产VIM-2型金属β-内酰胺酶,1株产IMP-1型金属β-内酰胺酶,未检测到IMP-2和VIM-1。鲍曼不动杆菌在体外可被多粘菌素B诱导成耐药菌株。碳酰氰基对氯苯腙(CCCP)可显著降低诱导耐药株多粘菌素B的MIC值。鲍曼不动杆菌诱导前后PmrA-PmrB基因序列完全相同。 结论: 碳青霉烯耐药鲍曼不动杆菌中,绝大部分菌株(95.9%,94/98)产OXA-23,将近一半的菌株(41/98)产VIM-2,个别产IMP-1。体外可诱导鲍曼不动杆菌对多粘菌素B耐药,耐药原因可能与外排泵的活化有关。
[Abstract]:Objective: to detect 尾-lactamase gene in acinetobacter baumannii resistant to carbapenem. To study the resistance mechanism of Acinetobacter baumannii to polymyxin B induced in vitro. Methods: 98 strains of carbapenem resistant Acinetobacter baumannii were collected from 8 hospitals in Zhejiang Province and 1 hospital in Henan Province. The minimal inhibitory concentration of 14 antimicrobial agents was determined by Agar dilution method and E-test method. The homology of the strains was analyzed by pulsed field gel electrophoresis (PFGE). The common carbapenase gene was amplified by PCR. The resistance of Acinetobacter baumannii to polymyxin B was induced in vitro, and the mechanism of resistance of Acinetobacter baumannii to polymyxin B was investigated. Results: 98 strains of Acinetobacter baumannii were resistant to many antibiotics including carbapenem and were all sensitive to polymyxin B. The drug resistance rates of tegacycline, amikacin and ciprofloxacin were 88.8% and 97.4%, respectively. The resistance rates of the other 10 antibiotics were 100%. The PFGE bands of 48 strains selected according to the phenotype of drug resistance were the same or similar to those of the same clone or closely related strains. Out of 98 strains, 94 strains produced OXA-23 type carbapenem and 41 strains produced VIM-2 type metal 尾 -lactamases. 1 strain producing IMP-1 type metal 尾 -lactamases, IMP-2 and VIM-1. were not detected Acinetobacter baumannii can be induced into resistant strains by polymyxin B in vitro. Carbyl cyanoyl p-chlorophenylhydrazone (CCCP) significantly decreased the MIC value of polymyxin B in the induced drug resistant strain. The PmrA-PmrB gene sequence of Acinetobacter baumannii was identical before and after induction. Conclusion: in acinetobacter baumannii resistant to carbapenem, most of the strains (95.9% or 94 / 98) produce nearly half of OXA-23, (41 / 98) producing VIM-2, and producing IMP-1. individually. Acinetobacter baumannii resistance to polymyxin B could be induced in vitro.
