人博卡病毒在健康献浆人群中的基因组和血清学抗体流行情况研究

【摘要】:目的了解中国健康献浆人群、原料血浆和血液制品中人博卡病毒(Human bocavirus, HBoV)的基因组和血清学抗体的流行情况以及遗传多样性特征。方法基于ABI7900HT Fast Real-Time PCR System平台,建立检测HBoV DNA的特异性、敏感性Real-Time PCR (SYBR Green法)方法。并根据此方法对从血液制品公司收集到的健康献浆人群的单人份血浆(不同月份共6096份)以及本实验室中保存的原料血浆(共241份)和血液制品[共326份,包含白蛋白65份,静脉注射丙种球蛋白(intravenous immunoglobulin,IVIG)197份,凝血因子Ⅷ(factor Ⅷ,FⅧ)47份,纤维蛋白原(fibrinogen, Fg)12份,蛋白酶原复合物(prothrombin complex concentrate, PCC)5份]进行HBoV DNA的筛查,并用Nested-PCR的方法进行确认。巢式确认阳性后样本进行820bp片段的巢式PCR扩增、测序并进行遗传多样性分析。同时,对209份血浆样品(根据HBoV病毒载量分为3组:组1,病毒载量104copies/mL;组2,基因组阳性但病毒载量104copies/mL;组3,基因组阴性)分别进行血清学抗HBoVIgG和IgM抗体的筛查。 结果HBoV在健康献浆人群中的基因组流行率为9.06%,病毒载量范围为5.01×102-3.02×106copies/mL。241份原料血浆中有43份有BoV DNA检出,检出率为17.84%,其中3份病毒载量高于104copies/mL。326份血液制品中44份为HBoVDNA阳性,阳性率为13.50%,其中2份白蛋白(3.08%,2/65)、14份IVIG(7.11%,14/197)、26份FⅧ(55.32%,26/47)和2份Fg(16.67%,2/12)中HBoV DNA阳性,5份PCC中未检出HBoV DNA。不同病毒载量血浆中血清学抗体阳性率分析表明,HBoV核酸检测阳性血浆中抗HBoV IgG抗体检出率高于HBoV核酸阴性血浆,HBoV病毒高载量组血浆中抗HBoV IgM抗体检出率高于HBoV阴性组。遗传多样性分析显示在中国被检地区健康献浆人群中流行的HBoV为1型。 结论1型人博卡病毒在中国健康献浆人群、原料血浆和血液制品中存在较高流行率,但大多处于低载量流行。HBoV是否对血液安全构成威胁,尚有待进一步研究。
