针刀干预椎动脉型颈椎病模型兔的实验研究

【摘要】:目的:观察针刀干预椎动脉型颈椎病(cervical spondylosis of vertebral artery type,CSA)模型兔肌肉组织光镜下的形态学变化和血清一氧化氮(nitric oxide,NO)、血浆内皮素(endothelin,ET)含量的影响,探讨针刀干预椎动脉型颈椎病的部分机制。方法:选用雌性新西兰家兔25只,取5只作为空白对照组,剩余造模成功的15只随机分为模型组、电针组、针刀组,各5只。空白组、模型组不干预;电针组穴取"百会""风池""颈1~3夹脊",针刺得气后,连接华佗牌SDZ-Ⅱ型电子针灸仪,每次20 min,每天1次,10天为一疗程,疗程间休息1天,共2个疗程;针刀组在C_3~C_5棘突旁开1.5~2 cm处予针刀治疗,7天1次,共3次。观察各组家兔颈部肌肉组织形态学变化,应用硝酸还原酶法检测各组血清NO的含量,采用放射免疫法检测各组血浆ET的含量。结果:与电针组相比,光镜下观察针刀组干预后针切部位肌细胞轻度坏死,间质间未见明显的炎性细胞浸润,针切断裂部位趋向于正常骨骼肌及筋膜结构结缔组织修复。模型组血清NO含量低于空白对照组(P0.01),血浆ET含量高于空白对照组(P0.01);电针组与针刀组血清NO含量高于模型组(均P0.05),血浆ET含量低于模型组(均P0.05);针刀组家兔血浆ET含量明显低于电针组(P0.05)。结论:针刀干预通过促进炎性细胞消散,改善肌肉组织修复,提升血清NO含量,降低血浆ET含量,使椎动脉维持在一种正常的生理舒张状态,保证了脑组织的正常灌注,可能是针刀干预治疗CSA的部分作用机制。
