右安门社区家庭医生服务制度完善研究

【摘要】：2010年,北京市在全国率先开展家庭医生式服务的试点工作。2011年3月11日,《北京市社区卫生家庭医生式服务工作方案》出台,标志北京市的基层医疗机构的改革在全国的示范和引领。同时随着医改的不断深入,时代又赋予了家庭医生式服务全生命周期健康管理、医养结合等新的服务内涵。2016年4月,中央深化改革领导小组会议审议通过了《关于推进家庭医生签约服务的指导意见》,签约服务将成为2016年及之后一个时期我国医药卫生体制改革的基础性工作。右安门社区卫生服务中心,2010年成为北京市家庭医生式服务的首批试点单位。5年来,社区以家庭医生为核心,以家庭医生服务团队为支撑,通过签约的方式促使家庭医生与签约居民建立长期、稳定服务关系,从而为签约居民提供连续性、协调性、综合性基本医疗和公共卫生服务的新型社区卫生服务模式,基本实现大部分居民的健康问题可以在基层医疗卫生机构得到解决的阶段性目标。本研究以北京市丰台区右安门社区卫生服务中心的社区实践为个案,以公众对家庭医生认知为研究视角,通过对家庭医生式服务的供方、需方角度进行调查,了解北京市丰台区社区卫生服务中心家庭医生式服务工作运行五年来的发展现状,研究和分析目前其运行中存在的不足与问题,主要有高素质人力资源不足,服务能力与服务功能定位离社区居民的期望存在差距、区域协同服务机制尚需完善、缺乏建设高效的信息化平台、医保等配套政策联动效应不明显等。并结合国内外家庭医生式服务模式运行状况,提出进一步改善家庭医生式服务的措施,健全完善医养护一体化家庭医生式服务的新模式。同时,为北京市社区卫生服务机构开展家庭医生式服务提供理论参考,为相关部门制定符合本地区实际的政策意见提供科学的理论支持。本研究对进一步推动北京市家庭医生式服务内涵的建设和社区卫生服务的深化改革具有一定的现实意义。
