京津冀地区基本医疗卫生服务均等化研究

【摘要】：医疗卫生服务的普及程度关系到全体居民的基本健康,能否保障居民的身体健康关系到整个社会是否稳定和健康发展,因此推动基本医疗卫生服务均等化是当前重要的目标,并且一直以来也是中央和地方关注的热门话题。京津冀协同发展这一战略很早就被提出,直至2014年习近平总书记第一次把京津冀协同发展上升为国家战略。当前,京津冀地区是我国三大都市圈之一,打造环首都新的经济圈,成为京津冀地区协同发展的战略之一,并且把京津冀地区医疗一体化作为京津冀地区协同发展的突破口之一。所以,京津冀地区基本医疗卫生服务均等化的研究对促进社会经济协调、健康发展,实现京津冀协同发展有重要的现实意义。因此在这样的时代背景下,选择京津冀地区为研究对象,分析其基本医疗卫生服务均等化水平及影响其均等化水平的因素,为实现京津冀地区基本医疗卫生服务均等化提出建议。本文首先利用描述性统计方法分析京津冀地区基本医疗卫生资源的现状,并且运用基尼系数分析其均等化水平;其次构建基本医疗卫生服务指标,应用熵值法、泰尔指数、熵权TOPSIS法对京津冀地区基本医疗卫生服务均等化水平进行评价分析,结果可以看出:京津冀地区基本医疗卫生服务总体的非均等化程度在逐渐减小,对其基本医疗卫生服务水平二级指标分析可知,京津冀地区的医疗卫生物力与人力资源,医疗卫生服务利用和居民健康水平这三个方面的非均等化程度较高,在医疗卫生财力资源、医疗卫生服务质量和效率这两个方面的非均等化水平有所改善;然后找出可能影响京津冀地区基本医疗卫生服务不均等的原因,并运用面板回归、脉冲响应、方差分解方法进行实证研究;最后对实现京津冀地区基本医疗卫生服务均等化提出有益的建议。
[Abstract]:The popularity of medical and health services is related to the basic health of the whole population, and whether the health of the residents can be guaranteed is related to the stability and healthy development of the whole society. Therefore, it is an important goal to promote the equalization of basic medical and health services. And has always been the central and local attention of the hot topic. The Beijing-Tianjin-Hebei coordinated development strategy was put forward early on until 2014, when President Xi Jinping for the first time upgraded the co-development of Beijing, Tianjin and Hebei to a national strategy. At present, the Beijing-Tianjin-Hebei region is one of the three metropolitan areas in China. Building a new economic circle around the capital has become one of the strategies for the coordinated development of the Beijing-Tianjin-Hebei region. And Beijing-Tianjin-Hebei region medical integration as one of the Beijing-Tianjin-Hebei region coordinated development breakthrough. Therefore, the study of equalization of basic medical and health services in Beijing-Tianjin-Hebei region has important practical significance for promoting social and economic coordination, healthy development and realizing the coordinated development of Beijing-Tianjin-Hebei. Therefore, under the background of this era, the Beijing-Tianjin-Hebei region is chosen as the research object to analyze the equalization level of basic medical and health services and the factors affecting its equalization level. To realize the equalization of basic medical and health services in Beijing, Tianjin and Hebei. This paper first uses descriptive statistical method to analyze the current situation of basic medical and health resources in Beijing-Tianjin-Hebei region, and uses Gini coefficient to analyze its equalization level. The entropy weight TOPSIS method is used to evaluate the equalization level of basic medical and health services in Beijing-Tianjin-Hebei area. The results show that the degree of non-equalization of basic medical and health services in Beijing-Tianjin-Hebei region is gradually decreasing. The analysis of the secondary indexes of the basic medical and health service level shows that the non-equalization degree of the three aspects of the medical and health resources and human resources, the utilization of the medical and health services and the health level of the residents in the Beijing-Tianjin-Hebei region is relatively high. The non-equalization level of medical and health resources, the quality and efficiency of medical and health services has been improved, and then the reasons that may affect the inequality of basic medical and health services in Beijing, Tianjin and Hebei regions have been found out, and the panel regression has been used. The impulse response and variance decomposition method are studied empirically. Finally, some useful suggestions are put forward to realize the equalization of basic medical and health services in Beijing-Tianjin-Hebei region.
