应用螺旋水刀行面神经解剖术的实验研究

【摘要】: 目的 本研究旨在探讨一种理想水压的螺旋水刀在面神经解剖术中的应用。材料和方法 实验动物为12只杂种狗,将其分为A、B两组,每组6只。A组全部实验狗均使用喷嘴直径为120μm的螺旋水刀行保留面神经的双侧腮腺全切术,左侧腮腺选用30-50 bar水压,右侧腮腺选用50-60 bar水压。B组左侧腮腺选用50-60bar水压,喷嘴直径为120μm的螺旋水刀施术,右侧腮腺选用常规手术器械施术,双侧腮腺均行保留面神经的双侧腮腺全切术。 A、B两组观察指标相同。术前、术后观察实验狗的面神经功能情况;术前、术后行面神经电生理检测;术后2周取面神经标本,行组织学观察。 结果 实验一(A组)结果:术前、术后所有实验狗均未出现明显面瘫症状;行术后30-50 bar水压组和50-60 bar水压组的面神经传导速度比较,差别无统计学意义(P>0.05);行组织学观察,两组的神经损伤均介于Ⅰ-Ⅱ级。 实验二(B组)结果:术后常规手术器械组的部分实验狗出现局部面瘫症状;行术后50-60 bar水压组和常规手术器械组的面神经传导速度比较,差别有统计学意义(P<0.05),前者速度大于后者;行组织学观察,50-60 bar水压组的神经损伤介于Ⅰ-Ⅱ级,常规手术器械组的神经损伤介于Ⅰ-Ⅳ级。 结论 本实验证实:应用螺旋水刀行面神经解剖术可减小对面神经的损害,较早恢复面神经功能。
[Abstract]:Purpose. The purpose of this study was to investigate the application of an ideal water pressure spiral water knife in facial nerve dissection. Materials and methods. Twelve mongrel dogs were divided into two groups: group A (n = 6) and group A (n = 6). All dogs in group A were treated with a spiral water knife with a nozzle diameter of 120 渭 m. The left parotid gland was treated with 30 to 50 bar water pressure. In the right parotid gland, 50-60 bar water pressure was used in the left parotid gland, and the diameter of the nozzle was 120 渭 m in the left parotid gland. The right parotid gland was performed with conventional surgical instruments. The bilateral parotid gland was treated with total bilateral parotid gland with facial nerve preservation. The function of facial nerve in experimental dogs was observed before and after operation, the electrophysiological examination of facial nerve was performed before and after operation, and the specimens of facial nerve were taken 2 weeks after operation for histological observation. Results. Results: there were no obvious symptoms of facial paralysis in all dogs before and after operation, and there was no significant difference in the conduction velocity of facial nerve between the 30 to 50 bar water pressure group and 50-60 bar water pressure group (P > 0.05).
