急诊护理硕士专业学位研究生临床实践模式的研究

【摘要】:目的:本课题主要探索目前我国护理硕士专业学位研究生的研究方向设置情况,同时调查其临床实践情况,分析研究生临床实践中存在的一系列问题,在此基础上,构建出具有中国特色的急诊护理硕士专业学位研究生临床实践模式,各高校可参照此模式对急诊护理专业学位研究生进行统一培养,以解决目前护理研究生教育水平参差不齐的问题,最终提高临床护理服务质量。对象:收集2016年招收护理硕士专业学位研究生的86所院校的招生信息,包括招生简章、专业目录以及导师研究方向,整理分析招生信息,总结目前我国设置的护理硕士专业学位研究生的研究方向;选取全国8所院校76名护理硕士专业学位研究生进行问卷调查,了解目前护理硕士专业学位研究生临床实践现况;根据专家的入选标准,综合考虑专家代表性、研究领域多样性等多种因素的存在,最终选取28名护理专家进行专家函询。方法:了解我国护理硕士专业学位研究生的研究方向设置情况主要通过收集整理2016年招收护理硕士专业学位研究生的院校招生信息来实现;了解我国护理硕士专业学位研究生临床实践情况及其存在的问题主要运用的研究方法是文献分析法、半结构式访谈法以及问卷调查法;运用文献分析法、半结构式访谈法,初步拟定急诊护理硕士专业学位研究生临床实践模式的指标体系;通过德尔菲专家函询法及统计学方法,对指标进行筛选与修改,最终确立急诊护理硕士专业学位研究生临床实践模式。结果:1.2016年招收护理硕士专业学位研究生的86所高校中,20所高校明确表示不区分研究方向,10所高校的研究方向仅仅是护理临床技能应用与研究或临床护理技能训练与研究;在区分研究方向的56所高校中,其中10所的研究方向为护理教育、护理管理、临床护理、社区护理等大概方向,其余46所将研究方向细化为外科护理、内科护理、急危重症护理等专科护理方向,这与我国国务院规定的护理硕士专业学位的培养目标基本一致。2.所有护理硕士专业学位研究生均参与临床实习,且实习医院均为三级甲等医院。大部分院校临床实践轮转科室为全科轮转,没有根据研究生的研究方向选择特色轮转科室;关于研究方向,32.89%的学生选择临床护理,其中9人选择急诊急救护理方向;42.11%的研究生临床实践总时间为12个月以内,临床实践总时间偏少;80.26%的研究生所在院校没有明确规定实践内容与要求;临床带教师资水平参差不齐,带教方式与本科生没有区别或区别不大,难以满足研究生的学习需求;考核评价方式比较单一,没有制定出切实可行的统一标准。3.两轮专家咨询的问卷有效回收率分别为90.32%和100%,可见,各位专家对本课题的研究持支持态度,并具有浓厚兴趣;专家权威系数为0.879,表明各位专家是根据其理论知识和实践经验来做出判断的,并非凭空想象,可信度较高;肯德尔协调系数为0.314(c2=1370.602,p0.01),表明专家预测协调性比较好。根据指标筛选标准,同时结合专家意见对指标进行筛选及修改。本课题共经过两轮专家函询,最后构建的急诊护理硕士专业学位研究生临床实践模式包含4个一级指标,17个二级指标和136个三级指标。结论:目前我国各院校根据国家教育部的总体要求制定适合本院校的专业型护理研究生临床实践模式,这导致各院校培养的研究生质量参差不齐,严重影响我国护理教育事业的发展。因此,构建科学、全面、规范的专业型护理研究生临床实践模式,有助于提升护理教育质量,提高临床护理水平,满足医院对高级护理人才的需求。本课题以临床需求为导向,以岗位胜任力为核心制定出急诊护理硕士专业学位研究生临床实践培养模式,以培养一批高素质的急诊专科护理队伍,以满足当今社会对高水平急救护理的需求,同时为各高校培养和教育急诊护理硕士专业学位研究生提供一定的参考依据。
[Abstract]:Objective: To explore the research direction of graduate students of nursing master's degree, investigate the clinical practice and analyze a series of problems in the clinical practice of graduate students, and on this basis, build a clinical practice model of postgraduate students with Chinese characteristics. Colleges and universities can refer to this model to unify the graduate students of emergency nursing degree, in order to solve the problems of the current level of nursing graduate education, and ultimately improve the quality of clinical nursing services. Object: to collect the enrollment information of 86 colleges and universities for postgraduate nursing master's degree in 2016, including the enrolment chapter and the professional list. Record and tutor research direction, collate and analyze enrollment information, summarize the research direction of graduate students of nursing master's degree set up at present in our country, select 76 graduate students of master of nursing degree in 8 colleges and universities to carry out a questionnaire survey to understand the current situation of graduate students' clinical practice in nursing master's degree; according to the criteria of the experts' admission Considering the existence of many factors such as expert representation and diversity in the field of research, 28 nursing experts are finally selected to consult with experts. Methods: to understand the research direction of graduate students of nursing master's degree in our country mainly through collecting and sorting out the enrollment information of graduate students of nursing master's degree in 2016. At present, the main methods of studying the clinical practice of postgraduate nursing graduate students in China and their existing problems are literature analysis, semi structural interview and questionnaire survey, and using the method of literature analysis and semi structural interview to preliminarily draw up the indexes of the clinical practice model of the postgraduate students in emergency nursing master's degree. System; through the Delphy expert inquiry method and statistical method, the index is screened and modified, and the clinical practice model of the master degree graduate student of emergency nursing is established. Results: in the 1.2016 years, 20 colleges and universities in the 86 colleges and universities of the master degree of nursing master degree show that the research direction is indistinguishes, and the research of the 10 colleges and Universities To only nursing clinical skills application and research or clinical nursing skills training and research; of the 56 colleges and universities in which the research direction is divided, 10 of them are in the direction of nursing education, nursing management, clinical nursing, and community nursing, and the other 46 are divided into surgical nursing, medical care, emergency care and so on. Special nursing direction, which is basically the same as the training goal of the master of nursing professional degree stipulated by the State Council of China.2. all the students of nursing master's degree are all involved in clinical practice, and the practice hospitals are all three grade A hospitals. Therefore, the construction of a scientific, comprehensive and standardized clinical practice model of professional nursing postgraduates is helpful to improve the quality of nursing education, improve the level of clinical nursing, and meet the high level of nursing care in the hospital. In order to meet the needs of the high level emergency nursing care in the present society, and to nurture and educate the emergency nursing care for all colleges and universities, this paper takes the post competency as the core to formulate the training mode of the postgraduate students' clinical practice in emergency nursing, in order to train a group of high quality emergency nursing teams to meet the needs of the high level emergency care in the present society. It provides some reference for Postgraduates of master's degree.
